Abstract :
In this column, the authors propose a practice-based clinical pathway development process using patient data from electronic health records. The authors aim to examine the interaction between patients´ health conditions and treatment approaches, and learn the most common pathways of care to inform better patient engagement and outcomes in healthcare practices.
