Abstract :
The application of health information technology can help staffs of long-term care institution to arrange daily care of residents, and provide better personalized care. In this study, we developed a classification and grading system (CGS) for resident assessment which grades individuals in terms of the severity of five functional status assessment aspects of mental, movement, eating, toilet (urination and defecation function) and medical treatment. By the aid of CGS, residents can be classified into different care types, and the assessment result can assist care professionals to facilitate their care work and to deliver proper care to individual needs. We designed and built CGS by UML methodology for long-term care institution. With the systematic appraisal, we can not only help the care personnel to know care needs of the residents, but also make individual care plan and care weekly schedule, so as to achieve the aims of comprehensive assessment and personalized care. Moreover, the residents´ care quality care personnel´s work efficiency and institution´s management efficiency can be all improved as the result of the completion of the CGS.
