تاثير استقرار طرح تحول نظام سلامت بر پرداخت رسمي بيماران بخش قلب: مطالعه موردي

Impact of Health System Reform on Patients’ Formal Payment in Heart department: Case Study

مقدمه: سلامت از جمله حقوق طبيعي و نيازهاي اساسي انسان ها بوده و به عنوان يكي از پيش شرط هاي اصلي نظام هاي رفاه اجتماعي شناخته شده است. با اين وجود نبود محافظت مالي در سلامت به عنوان يكي از بيماري هاي نظام هاي سلامت شناخته شده است. پس از روي كار آمدن دولت يازدهم، وزارت بهداشت، درمان و آموزش پزشكي طرح تحول سلامت را به دنبال محافظت مالي از مردم تدوين كرد. روش ها: پژوهش حاضر، مطالعه اي مقايسه اي از نوع مقطعي مي باشد. جامعه آماري پژوهش صورتحساب كليه بيماران بخش قلب مركز در بازه زماني مشخص شده بودند. با استفاده از جدول مورگان و روش نمونه گيري تصادفي طبقه اي به ترتيب تعداد 120 و 154 نمونه از پرونده هاي بخش قلب در سال هاي 92 و 93 انتخاب شد. داده هاي مورد نياز از صورتحساب مالي بيماران استخراج و با استفاده از روش آماري t-test و من ويتني تحليل شد. براي تحليل داده ها از نرم افزار SPSS v.22 استفاده گرديد. يافته ها: با افزايش زياد تعرفه هاي خدمات درماني طي اجراي طرح، ميزان پرداخت رسمي بيماران كاهش چشمگيري نداشته است. همچنين، سازوكارهاي بيمه اي مانع از رسيدن طرح تحول به هدف كاهش ميزان پرداخت مستقيم بيمار شده است. نتيجه گيري: نتايج مطالعه بيانگر تاثير عوامل مختلفي همچون سازوكارهاي بيمه اي، تعرفه خدمات درماني، نوع بيمه فرد، سرپايي يا بستري بودن مي باشد؛ بنابراين، شايسته است دولت نتايج حاصل از مطالعه را در برنامه ريزي به منظور افزايش پوشش و محافظت مالي مردم به كار گيرد.

Introduction: Health is one of the natural rights and basic human needs and is recognized as one of the prerequisites for social welfare systems. However, Lack of financial health protection is recognized as one of the diseases in health systems. When the eleventh government get start, the Ministry of Health developed the health sector reform to seek financial protection from the people. The purpose of this study was to investigate the effect of Health System Reform on Patients’ Formal Payment in Heart Sector of Qazvin’s velayat Hospital. Methods: This was a comparative study of discontinuous time series. The bills of all patients referring to the heart department during the specified period formed the statistical population of the study. By using Morgan table and stratified random sampling, 120 and 154 samples from cardiac files were selected in the years 2013 and 2014 respectively. Data were extracted from patient's financial bills. Finally, data was analyzed by t-test and SPSS software. Results: With the increase in health care tariffs during the plan implementation, the official payment of patients has not declined significantly, and new insurance mechanisms have prevented the development plan from reaching the goal of reducing the patient's direct payment to six percent of the bill. Conclusion: Results of the study indicate the impact of various factors such as insurance mechanisms, health care tariffs and kind of patient insurance. Therefore, it is worth the government to use results of the study in planning to increase coverage and financial protection of the people
