بررسي تاثير انحراف سپتوم بيني در ايجاد سينوزيت مزمن

Evaluation of the effect of nasal septum deviation on chronic sinusitis

مقدمه: ابتلا به سينوزيت مزمن در اثر اختلالات ميكرو آناتوميك خصوصاً راه‌هاي هوايي باريك و نواحي تماس مخاطي در بيني و سينوس‌ها ايجاد مي‌شود. از جمله عوامل زمينه‌ساز مي‌توان از انحراف سپتوم بيني نام برد. از اين‌ رو هدف اين مطالعه تعيين فراواني نسبي شدّت سينوزيت در بيماران دچار انحراف بيني، تأثير سن افراد دچار انحراف بيني در شدّت سينوزيت مزمن و تأثير شـدّت انحراف بيني در شـدّت سينوزيت مزمن مي‌باشد. مواد و روش‌ها: در اين مطالعه مقطعي- توصيفي 109 فرد بالغ با سينوزيت مزمن كه داراي راديوگرافي واترز و سي تي اسكن بودند مورد بررسي قرار گرفتند. در اين بيماران وجود انحراف تيغه‌ي بيني و عدم وجود فاكتورهاي مستعد كننده‌ي ديگر سينوزيت محرز بود. اطلاعات با استفاده از آزمون Spearman و آزمون همبستگي تجزيه و تحليل گرديد. (0/05=α) يافته‌ها: همه‌ي بيماران علاوه بر سينوزيت داراي انحراف بيني بودند كه نشان دهنده‌ي ارتباط قوي بين انحراف بيني و ايجاد سينوزيت مزمن مي‌باشد. بيشترين فراواني مربوط به گروه سني 29-20 سال و كم‌ترين فراواني مربوط به گروه بالاي 50 سال بود. بين افزايش شدت سينوزيت و افزايش شدت انحراف تيغه‌ي بيني با افزايش سن ارتباط معناداري وجود نداشت. نتيجه‌‌گيري:با توجه به محدوديتهاي اين مطالعه ارتباط قوي بين افزايش شدت انحراف بيني و شدت سينوزيت وجود داشت.

Introduction: Chronic sinusitis results from micro-anatomic disturbances, especially from narrow airways and mucous contact areas in the nose and sinuses. One of the predisposing factors is the nasal septum deviation. Therefore, the aim of the present study was to determine the relative frequency of sinusitis severity in patients with nasal septum deviation, the effect of age in patients with nasal septum deviation on the severity of sinusitis, and the effect of nasal septum deviation severity on sinusitis severity. Materials and Methods: In this descriptive, cross-sectional study 109 adult subjects with chronic sinusitis, who had Water’s radiographic views and CT scans were evaluated. These patients had nasal septum deviation with no other predisposing factors for sinusitis. Data was analyzed with Spearman’s correlation coefficient test (α = 0.05) . Results: All the subjects with sinusitis had nasal septum deviation, which demonstrates a strong correlation between chronic sinusitis and nasal septum deviation. The highest and lowest frequencies were observed in the 20-29 and >50 age groups, respectively. There was no significant relationship between sinusitis severity and nasal septum deviation severity on one hand and aging on the other. Conclusion: Under the limitations of the present study it was concluded that there was a significant correlation between nasal septum deviation severity and sinusitis severity.
