بررسي تغييرات آگاهي و نگرش بيماران مبتلا به اختلالات عروق كرونر نسبت به بيماري خود در طول بستري در بخشccu بيمارستان وليعصر (عج) بيرجند

Changes in awareness and attitude toward their disease in patients with acute coronary syndrome admitted to the CCU of Vali-Asr Hospital -Birjand

زمينه و هدف: پيش بيني مي شود سندرم كرونري حاد تا سال 2030 علت مرگ 23/4 ميليون نفر در جهان باشد. تعديل سبك زندگي به وسيله آموزش بهداشت در زمينه آگاهي از عوامل خطر و تغيير نگرش بيماران؛ عامل مهمي در بهبود پيش آگهي آن است. اين مطالعه با هدف تعيين تغييرات آگاهي و نگرش بيماران مبتلا به اختلالات عروق كرونر نسبت به بيماري خود در مدت زمان بستري در بخش CCU، انجام شد. مواد و روش ها: در اين مطالعه توصيفي كه در بخش CCU بيمارستان وليعصر (عج) بيرجند انجام شد، بيماران به روش سرشماري مورد بررسي قرار گرفتند. آگاهي و نگرش بيماراني كه در فاصله زماني 1/5 ماه در بخش CCU بستري مي شدند و به شيوه معمول آموزش مي ديدند، يك بار در روز اول بستري و يك بار هم در زمان ترخيص و همچنين كميت آموزش به آنان در زمان ترخيص از بخش اندازه گيري شد. اطلاعات لازم با استفاده از پرسشنامه محقق ساخته در خصوص آگاهي و نگرش و كميت آموزش بيمار جمع آوري شد. روايي اين پرسشنامه به روش اعتبار محتوي و نظر اساتيد و پايايي آن با استفاده از آزمون آلفاي كرونباخ محاسبه گرديد. براي تجزيه و تحليل داده هاي كمي از آمار توصيفي و براي مقايسه ميانگين هاي روز اول بستري و روز ترخيص از آزمون t استفاده شد. يافته ها: يافته هاي به دست آمده حاكي از اين است كه آگاهي بيماران در روز آخر بستري در مقايسه با روز اول افزايش معني دار نداشته است. نگرش بيماران در زمينه شدت درك شده فردي براي ابتلا مجدد، در سطح متوسط و بالا قرار داشت. ميانگين زمان آموزش به بيمار از طرف پرستاران طي روزهاي بستري 2/87 ± 3/83 دقيقه بود. نتيجه گيري: با توجه به نتايج به دست آمده در اين مطالعه، براي ارتقاء و بهبود سطح دانش و نگرش بيماران، بايد برنامه آموزش به بيمار گسترده تر شود. همچنين با توجه به اين كه 3/83 دقيقه زمان بسيار كمي براي آموزش به بيمار در طي دوره بستري در CCU مي باشد، پيشنهاد مي گردد مدت زمان بيشتري به اين امر اختصاص داده شود.

Background and objectives: It is predicted that 23.4 million people will be died of acute coronary syndrome up to 2030. Health education based on the awareness of risk factors of cardiovascular diseases and changing of patients' attitude of patients are among important factors affecting the prognosis of these diseases. This study was conducted to determine the changes of awareness and attitudes of patients with acute coronary syndrome toward their disease during their hospitalization in CCU. Materials and Methods: In this descriptive study, 60 patients admitted to the CCU of Vali-Asr Hospital of Birjand were included using census method. Patients' awareness and attitude were measured at the times of admission and discharge using a researcher-made questionnaire. Validity and reliability of the questionnaire was assessed using content validity and Cronbach's alpha test respectively. The quantitative data was analyzed using descriptive statistics and t test was used for comparison of the means of the dates of admission and discharge. Results: The findings of this study indicated that the rate of patients' awareness at the discharge time was not significantly increased compared to the corresponding value of the admission time. The patients' attitude toward the intensity of the perceived risk for recurrent morbidity was equals to or greater than the intermediate level. The average time of patients' education during their hospitalization by nurses was 3.83 ± 2.87 minutes. Conclusion: According to the results of this study, more and extensive training courses for patients are warranted to improve the level of their awareness and attitude toward the disease of acute coronary syndrome .
