تبيين درد رحم از منظر طب سنتي ايران و مقايسه با درد لگن از منظر طب رايج

Uterine pain explanation from Iranian Traditional Medicine point of view and comparison with pelvic pain from contemporary medicine review article

مقدمه: طب ‌سنتي ‌ايران رويكردي سيستماتيك و كل‌نگر دارد. درد رحم از منظر طب ‌سنتي ‌ايران به‌ معناي درد لگن با منشأ ژنيكولوژيك است. بسياري از علل ژنيكولوژيك درد لگن در طب رايج ناشناخته مي‌باشند. لذا مطالعه حاضر با هدف علت‌يابي موارد ناشناخته و بررسي علل درد رحم از منظر طب‌سنتي ‌ايران و مقايسه آن با طب رايج ‌انجام شد. روش‌كار: در اين مطالعه مروري، علل درد رحم از منابع مرجع طب‌ سنتي ‌ايران طي قرن‌هاي چهارم تا چهاردهم قمري بررسي ‌شدند. همچنين علل ژنيكولوژيك درد لگن از كتب مرجع ژنيكولوژي و 16 مقاله در پايگاه‌هاي اطلاعاتي جمع‌آوري ‌شدند. سپس علل درد رحم از منظر طب ‌سنتي ‌ايران با علل ژنيكولوژيك درد لگن از منظر طب رايج مقايسه ‌شدند. يافته‌ها: درد رحم از منظر طب ‌سنتي ‌ايران به دو دسته مرتبط با زمان خاص و غيرمرتبط با زمان خاص تقسيم مي‌شود. درد رحم مرتبط با زمان خاص با قاعدگي، مقاربت و زايمان ارتباط دارد. درد رحم غيرمرتبط با زمان خاص نيز داراي علل مختلف ساختماني، عملكردي و ساختماني - عملكردي است. درد لگن از نظر طب رايج به سه دسته حاد، سيكليك و مزمن تقسيم مي‌شود كه هر كدام داراي علل مختلفي مي‌باشند. نتيجه‌گيري: درد رحم مرتبط با زمان خاص از منظر طب‌سنتي با درد سيكليك لگن از منظر طب رايج و درد رحم غيرمرتبط با زمان خاص با درد مزمن لگن با منشأ ژنيكولوژيك مطابقت‌ دارد. مهم‌ترين تفاوت علل ژنيكولوژيك درد مزمن لگن بين طب سنتي ‌ايران و طب رايج، "سوء مزاج‌هاي تناسلي" و "نفخه رحم" است كه جزء علل عملكردي مي باشند و مي توانند يكي از علل ژنيكولوژيك شايع و چالش‌برانگيز درد مزمن لگن در طب رايج يعني علل ايديوپاتيك را توجيه‌ كنند.

Introduction: Iranian Traditional Medicine (ITM) has holistic and systematic approach. From ITM point of view, uterine pain is a pelvic pain with gynecological basis. Many gynecological causes of pelvic pain are unknown in the contemporary medicine. Therefore, this study was performed with aim to survey the causes of unknown cases and evaluate the causes of uterine pain from ITM point of view and compared with contemporary medicine. Methods: The current review study investigated the causes of uterine pain in ITM references from the fourth to the fourteenth centuries. Also, gynecological causes of pelvic pain were collected from gynecological textbooks and 16 articles from databases of Google Scholar, PubMed, Scopus and SID with keywords of chronic pelvic pain and pelvic pain. Then, the causes of uterine pain from ITM point of view were compared with the gynecological causes of pelvic pain from the contemporary medicine. Results: Uterine pain from ITM’s view is divided into two groups of related and unrelated to specific time. Uterine pain related to specific time is related to menstruation, coitus and delivery. Causes of uterine pain unrelated to specific time include structural, functional and structural-functional causes. From contemporary medicine, pelvic pain is divided into acute, cyclic and chronic that each has different causes. Conclusions: Uterine pain related to specific time from ITM's view is in accordance with cyclic pelvic pain from the contemporary medicine's view and uterine pain unrelated to specific time is in accordance with chronic pelvic pain with gynecological basis. The most important differences of gynecological causes of chronic pelvic pain between ITM and contemporary medicine are "genital dystemperaments" (Sui' e Mizaj) and "uterine distension" that are among the functional causes and can explain one of the most common and challengeable gynecological causes of chronic pelvic pain, the idiopathic causes, in the contemporary medicine.
