بررسي نياز‌هاي برنامه‌ي آموزشي پزشكان عمومي براي پاسخگو‌يي اجتماعي بر اساس مدل معادلات ساختاري

Evaluation of Requirements of Social Accountability in the Curriculum of General Practitioners Based on Structural Equation Models

زمينه و هدف: پاسخ­گويي اجتماعي در آموزش پزشكي به معناي خواستن و توانايي تطابق با نيازهاي بيماران و سيستم مراقبت از بيمار به صورت ملي و جهاني است. سرعت تغييرات در نظام سلامت و نيازهاي جامعه، بازنگري در آموزش پزشكي، به نحوي كه پزشكاني متناسب و آماده رويارويي با مشكلات قرن بيست و يكم تربيت كند، ضروري مي‌نمايد.از اين رو هدف پژوهش حاضر بررسي نيازهاي آموزشي پزشكان عمومي براي پاسخگويي اجتماعي مي­‌باشد. روش بررسي: روش پژوهش از نظر هدف كاربردي و از نظر شيوه‌­ي گردآوري داده­‌ها توصيفي از نوع همبستگي است. جامعه‌­ي آماري شامل صاحب نظران دانشگاه علوم پزشكي مشهد، اساتيد دانشكده‌­ي پزشكي، و پزشكان شاغل در سيستم بهداشتي درماني بودند. با توجه به اهداف تحقيق از روش نمونه گيري، غير تصادفي مبتني بر هدف بود. براي گردآوري داده­‌ها از پرسشنامه محقق ساخته استفاده شد. روايي پرسشنامه با استفاده از روايي محتوايي و تحليل عاملي تاييد و پايايي آن با استفاده از آلفاي كرونباخ 0/85 محاسبه شد. داده‌­ها با استفاده از نرم افزار SPSS و Amos تحليل شدند. يافته‌­ها: يافته‌­ها نشان داد بين مولفه‌­ي باليني و مولفه­‌ي پژوهش رابطه وجود دارد. بين مولفه‌­ي آموزشي و مولفه­‌ي باليني رابطه وجود دارد. بين مولفه‌­ي حمايتي و مولفه‌­ي آموزشي رابطه وجود دارد. بين مولفه­‌ي حمايتي و مولفه‌­ي باليني رابطه وجود دارد ولي بين مولفه­‌ي آموزش و مولفه­‌ي پژوهش و همچنين بين مولفه­‌ي حمايتي و مولفه­‌ي پژوهش رابطه معناداري يافت نشد. در نهايت اين كه بين پاسخگويي اجتماعي و حوزه‌­هاي آموزشي، باليني و پژوهشي رابطه­‌ي معناداري وجود دارد. نتيجه گيري: نتايج تحقيق بيان‌گر اهميت پرداختن به پاسخگويي اجتماعي در برنامه‌­هاي آموزشي پزشكان عمومي بر اساس مدل CARE به عنوان يك ابزار راهنما براي اجراي برنامه پاسخ­گويي است.

Background & Objectives: Social accountability in medical education is interpreted as the willingness and ability to adapt to the needs of patients and the healthcare system both nationally and globally. The speed of changes in the health system and the community’s needs has necessitated the training of physicians who are ready to face the problems of the 21st century. Therefore, this study aimed to evaluate the requirements of social accountability in the curriculum of general practitioners. Materials and Methods: This was an applied (in terms of goal) and descriptive and correlational (regarding data collection technique) research. The statistical population included experts of Mashhad University of Medical Sciences, professors of the school of medicine, and physicians working in the healthcare system. Subjects were selected through non-random purposive sampling. In addition, data were collected applying a researcher-made questionnaire, the validity of which was confirmed by content validity and factor analysis methods. Moreover, its reliability was estimated at Cronbach’s alpha of 0.85. Furthermore, data analysis was performed in SPSS and AMOS. Results: In this study, there was a relationship between the clinical and research components. In addition, an association was found between educational and clinical components, as well as between advocacy and educational components. On the other hand, while a relationship existed between the advocacy and clinical components, there was no significant association between educational and research components, as well as between advocacy and research components. In the end, a significant relationship was observed between social accountability and educational, clinical, and research fields. Conclusion: According to the results of this study, it is vital to focus on social accountability in the curriculum of general practitioners based on CARE model as a guide tool for the implementation of the accountability program.
