ميانگين ارتفاع، شكل و قطر قدامي-خلفي غده هيپوفيز در افراد نرمال 15 تا 40 سال

The mean height and anteroposterior diameter and shape of a normal pituitary gland in a sample population at the age group of 15-40

غده هيپوفيز يك غده درون ريز است و داراي وزن حدود0.5 گرم مي باشد. در واقع يك برآمدگي از كف هيپوتالاموس در كف مغز مي باشد.اندازه آن در جنس و گروه هاي سني مختلف متفاوت است و در بيماري هاي مختلفي تحت تاثير قرار ميگيرد.بنابراين دانستن اندازه هيپوفيز نرمال جهت تشخيص و تعيين پيش آگهي بيماري هاي آن ضروري به نظر مي رسد .دربررسي هاي ما مطالعه ثبت شده اي در ايران تحت عنوان اندازه گيري ابعاد هيپوفيز در جمعيت نرمال يافت نشد.در اين راستا تصوير برداري تشديد مغناطيسي(magnetic resonance imaging) يا همان MRI ، مناسب ترين وسيله براي بررسي غده هيپوفيز است.مطالعه حاضر با هدف اندازه گيري ارتفاع و قطر قدامي- خلفي غده هيپوفيز نرمال در زنان ومردان 15 تا 40 سالانجام گرفت. روش:پژوهش حاضر يك مطالعه مقطعي بود .119نفر از افراد مراجعه كننده به مركزMRIفاطمه زهرا(س) كرمان كه فاقد سابقه بيماري مرتبط با هيپوفيز وفاقد سابقه مصرف داروهاي موثر بر سطح سرمي پرولاكتين واستروئيد بودهو در آنها مواردي از قبيل حاملگي و شيردهي رد شده بود ،بصورت غير احتمالي آسانانتخاب شدند. و ارتفاع وقطر قدامي-خلفي هيپوفيز در آنها بوسيله تصوير برداري تشديد مغناطيسي اندازه گيري شد.داده ها با استفاده از نرم افزار spss19 و آزمونهاي تي تست و تحليل واريانس تجزيه و تحليل شد. نتايج:ميانگين ارتفاع هيپوفيز در زنان 1.2±5.72 و در مردان 1.28± 5.65 ميليمتر، كه در زنان بيشتراز مردان بوده ولي اين اختلاف معني دار نبود (p=0.7). بررسي گروه هاي سنينشان داد كه ارتفاع هيپوفيز در خانم ها با افزايش سن كاهش مي يابد و ميانگين اين ارتفاع در گروه سني 15 تا 20 سال بصورت معني دار از نظر آماري از دو گروه ديگر بيشتر بود (0.005 = p).اختلاف معني دار آماري بين ارتفاع هيپوفيز در گروه هاي سني آقايان مشاهده نشد (p= 0.37). ميانگين قطر قدامي خلفي در زنان 1.44±9.96 و در مردان 1.18±10.02 ميلي متر بود كه اختلاف آماري معني داري نشان نداد(p= 0.8) . در بيش از 50% موارد سطح فوقاني غده مسطح و در بقيه موارد محدب و يا مقعر بود. نتيجه گيري: اين مطالعه در نمونه اي از جمعيت سالم ايران در گروه هاي سني مختلف انجام شده و ميانيگن ها و نتايج به دست آمده از آن مي تواند به عنوان مرجع، در تشخيص پاتولوژي هاي اين غده حياتي بدن استفاده شود.

Introduction: The pituitary gland is an endocrine gland varying in size by gender and age group and based on the diseases affecting the individual. Knowing the normal size of a pituitary gland is therefore crucial in the diagnosis and prognosis of diseases of the gland. The present study was conducted to measure the normal height and anteroposterior diameter of the pituitary gland in men and women aged 15 to 40 in Kerman. Materials and Methods: The present cross-sectional study was conducted on 119 subjects admitted to Fatemeh Zahra Imaging Center in Kerman selected through non-probability convenience sampling and who had no history of diseases of the sellar and parasellar region, pituitary gland disorders (especially those requiring surgery) or taking medications affecting corticosteroid or prolactin levels and who were not pregnant or lactating. The height and anteroposterior diameter of participants' pituitary gland was measured by a mid-sagittal T1 sequence MRI. The data obtained were analyzed in SPSS-19 using the ANOVA and the independent t-test. Results: The mean height of the pituitary gland was 5.72±1.21 mm in the female subjects and 5.65±1.28 mm in the male subjects; however, the difference between the genders was not significant (P=0.7). The evaluation of the pituitary gland by age group showed that the height of the gland decreases in women by the increase in age and that its mean height is significantly higher in the 15-20 age group (p=0.005) compared to in the other age groups. There were no statistically significant differences in the height of the pituitary gland in men by age group (P= 0.37). The mean anteroposterior diameter was 9.96±1.44 mm in the female subjects and 10.02±1.18 mm in the male subjects, showing no statistically significant differences (P=0.8). The surface of the pituitary gland was flat in more than half of the cases, while it was concave or convex in the other cases. Conclusion: These measurements were performed on a healthy sample of the Iranian population from different age groups and can therefore be used as a point of reference for the evaluation of the pituitary gland in patients.
