بررسي بيان نشانگر HMWCK در هيپر پلازي خوش خيم وآدنو كا سينوماي پروستات با روش هيستو لوژي وايمونوهيستوشيمي

سابقه وهدف: در مورد ميزان بيا ن نشانگر High molecular weight cytokeratin در هيپرپلازي خوش خيم پروستات Benign prostatic hyperplasia و آدنو كا سينوماي پروستات اطلا عات ضد ونقيضي گزارش شده است هدف از اين مطالعه تعيين ميزان بيان نشانگرHMWCK در BPHوآدنوكارسينوماي پروستات باشاخص هاي آسيب شناسي و بافتي درغدد پروستات باروش ايمو نوهيستوشيمي مي‌باشد. مواد و روش ها: اين مطالعه توصيفي تحليلي بر روي تعداد 79 نمونه غدد خوش خيم BPH و آدنو كا سينوماي پروستات انجام شد. بعد از ثابت‌كردن نمونه‌ها در فرمالين، پاساژ بافتي و مقطع گيري انجام شد. سپس اسلايد ها با هما توكليسن وائوزين رنگ آميزي و نمونه هاي بدخيمي توسط پاتولوژيست ها بر پايه ي گليسون اسكور طبقه بندي شدند و براي بيان نشانگر HMWCK از روش ايمونوهيستو شيمي توسط دو نفر (دو سوكور) استفاده شد داده‌ها با آزمون كاي دو‌ تجزيه و تحليل شدند. يا فته ها: از 79 نمونه مورد مطالعه 43 مورد هيپر پلازي خوش خيم پروستات (BPH) و 36 مورد آدنوكارسينوماي پروستات بود. از 43 نمونه BPH 40 مورد از نظر نشانگر HMWCK مثبت و سه نمونه منفي بودند. از 36 نمونه ي آدنوكارسينوماي پروستات 35 نمونه از نظر بيان نشانگر HMWCK منفي ويك مورد مثبت بود. بيشتر نمونه هاي بدخيم در گليسون اسكور بالا قرارداشتند. از نظر آماري ارتبا ط معني داري بين نوع نمونه خوش خيم BPH و آدنو كار سينوماي پروستات با نشانگر HMWCK مشا هده شد. نتيجه گيري: بيان نشتانگر HMWCK در تشخيص دقيق و افتراقي بافت خوش خيم از آدنوكارسينوماي پروستات، همچنين پيش آگهي بيماري كمك كننده و مفيد شواهد بود.

Background and purpose The effect of the marker expression high molecular weight cytokeratin (HMWCK( in Benign prostatic hyperplasia (BPH) and adenocarcinoma prostate. There was conventional reported about rate expression HMWCK in adenocarcinoma prostate. The purpose of this studied was to determine expression of the marker HMWCK with histopathological parameters in BPH and adnocarcinoma prostates by immunohistochemistry methods. Materials & Methods This descriptive analytical research was conducted on 79 patients admitted to Hospital in Isfahan Iran. Samples were fixed in formalin and tissue processing and stained by hematoxilin and eosin adenocarcinoma samples were classified with Gleason pattern (score) method by pathologist. The effect of over expression in marker expression HMWCK was done after diagnosis by two expert (blind). The obtained data were analyzed using chi-square test. Results Out of the 79 specimens 43 samples were BPH and 36 samples were adenocarcinoma. 40 samples with diagnosis BPH were observed of the marker HMWCK and 3 samples were negative. Out of 36 samples with diagnosis adenocarcinoma, 35 over expression marker of the HMWCK were negative but one samples was positive. The over specimens malignancy were in higher Gleason score. There was significant relationships between type samples (BPH and adenocarcinoma) with over expression of the marker HMWCK p<0.001. Conclusion Our study to showed that usage of the marker HMWCK could be differentiated to increase diagnosis BPH tissue from cancer, therefore utilize of the marker HMWCK for prognosis prostate disease could be aided and benefit.
