تبيين تجارب پرستاران از صلاحيت‌هاي اخلاقي و باليني: يك رويكرد پديدارشناسانه

Nurses' Experiences about Ethical and Clinical Competencies: A Phenomenological Approach

زمينه و هدف: پرستاران بزرگ‌ترين گروه كاركنان بهداشتي را تشكيل مي‌دهند. در اين ميان توجه به صلاحيت‌هاي آن‌ها كه تضمين كننده كيفيت خدمات ارائه‌شده توسط آن‌ها مي‌باشد، از اهميت ويژه‌اي برخوردار است. از اين رو پژوهش حاضر با هدف توصيف و تفسير تجارب پرستاران در زمينه صلاحيت‌هاي اخلاقي و باليني آنان انجام شده است. مواد و روش‌ها: پژوهش حاضر ازنظر رويكرد جزء تحقيقات كيفي و از لحاظ روش، مبتني بر مطالعات پديدارشناسانه مي‌باشد. در اين راستا پس از نمونه‌گيري هدفمند، مصاحبه‌هاي نيمه‌ساختاريافته با 18 نفر از پرستاران در سال 1397 انجام گرفت، براي كسب اعتبار داده‌ها از معيارهاي Lincoln and Guba استفاده شد. داده‌ها بر اساس كدگذاري در سه سطح باز، محوري و منتخب مورد تجزيه ‌و تحليل قرار گرفتند. يافته‌ها: صلاحيت‌هاي پرستاران در 3 بُعد، 9 عامل و 22 مقوله؛ شامل بعد اخلاقي (رابطه با بيمار، اصول اجتماعي، تعهد ديني و تعهد حرفه‌اي)، بعد فردي (ويژگي‌هاي شخصيتي، انگيزش و توانايي ادراكي)، بعد تخصصي (دانش علمي و مهارت‌هاي تخصصي) پديدار شد. نتيجه‌گيري: بر اساس نتايج صلاحيت‌هاي مورد نياز يك پرستار را مي‌توان در سه بعد فردي، اخلاقي و تخصصي قرار داد. از اين رو برنامه‌ريزي آموزشي و مديريتي براي ارتقاي صلاحيت‌هاي اخلاقي و حرفه‌اي پرستاران مطابق با كدهاي اخلاق پرستاري، در جهت نهادينه‌كردن اين صلاحيت‌ها توصيه مي‌گردد.

Background and Aim: Nurses constitute the largest group of health care workers. In this regard, paying attention to their competencies, which guarantee the quality of services, is of particular importance. Therefore, the purpose of this study was to describe and interpret the experiences of nurses in the field of their moral and clinical competencies. Materials and Methods: The present study is a qualitative research with phenomenological approach. In this regard, after porposful sampling, semi-structured indepth interviews with 18 nurses were conducted, in 2018. Lincoln and Guba criteria were used to obtain data validity. The data were analyzed by coding in three open, axial and selected levels. Findings: Nurses competencies described in 3 dimensions, 9 themes and 22 sub-themes including ethical dimension (relationship with the patient, social principles, religious and professional commitment); individual dimension (personality traits, motivation and perceptual ability); specialized dimension (scientific knowledge and specialized skills). Conclusion: Based on the results, nurses competencies can be described into three dimensions: individual, ethical and specialized. Therefore, management and educational planning to improve the moral and professional compitencies of the nurses in accordance with the code of nursing ethics is recommended in order to institutionalize these competencies.
