چكيده فارسي :
زمينه و هدف: موفقيت چك ليست جراحي ايمن در كاهش مرگ و مير و عوارض جراحي، به طور گسترده‌اي وابسته به ميزان انطباق با چك ليست و اجراي صحيح اجزاي آن توسط تيم جراحي مي‌باشد. هدف از اين مطالعه تعيين چالشهاي موجود در اجراي مؤثر چك ليست جراحي ايمن و ارايه راهكارهايي براي اجراي مؤثرتر آن است.
روش بررسي: در مرور سيستماتيك حاضر كليه مقاله‌هاي كيفي مرتبط با اهداف مطالعه، چاپ شده از سال 2010 تا اكتبر 2018 بررسي شد. جستجو در پايگاه SID، Iran Medex، Iran doc، Magiran، PubMed/Medline، Web of Science، Scopus، ProQuest، Google Scholar، Cochrane library، Embase با كليد واژه‌هاي ايمني بيمار، فرآيند جراحي، چك ليست، سازمان جهاني سلامت، علم اجرا، پژوهش كيفي، Patient safety، Operative، Surgical Procedures، Checklist، World Health Organization، Implementation Science، Qualitative Research انجام يافت. معيارهاي ورود، مقاله‌هاي به زبان انگليسي يا فارسي، مقالات كيفي و مرتبط با اهداف مطالعه بود. معيارهاي خروج، مقاله‌هاي مروري، پوستر، سخن‌راني، نامه به سردبير و مطالعات كمي بود.
يافته‌ها: يافته‌هاي حاصل از بررسي 14 مطالعه كيفي نشان داد كه مهم‌ترين چالشهاي اجراي مؤثر چك ليست جراحي ايمن مربوط به اولويت‌هاي غيرقابل پيش‌بيني، عدم همكاري و هماهنگي اعضاي تيم جراحي، عدم تطبيق چك ليست با زمينه هر بيمارستان، عدم مشاركت بيمار و فقدان رويكرد برنامه‌ريزي شده در اجراي چك ليست بوده است. همچنين راهكارهاي بهبود اجراي چك ليست جراحي ايمن، عبارت است از بومي‌سازي چك ليست، بهبود همكاري و هماهنگي تمامي اعضاي تيم، آموزش و تمرين، مشاركت دادن بيمار و رهبري سازماني فعال.
نتيجه‌گيري: معرفي چك ليست جراحي ايمن در بخش مراقبت‌هاي سلامت يك چالش دايمي است و نيازمند ارزيابيهاي مستمر و ادغام آن با جريانهاي كاري موجود در بيمارستان، رهبري فعال و مؤثر، توضيح چرايي و چگونگي استفاده از آن توسط مديران و حمايت از جانب سازمان است. آموزش مداوم، ارزيابي عملكرد و مشاركت دادن تمامي اعضاي تيم جراحي در اجراي چك ليست، عوامل كليدي در اجراي مؤثر آن است.
چكيده لاتين :
Background & Aim: The success of the surgical safety checklist in reducing
surgical mortality and morbidity largely depends on the degree of compliance
with the checklist and correct implementation of its components by the staff.
The aim of this review is to determine the challenges of effective
implementation of the surgical safety checklist and to provide solutions for its
more effective implementation.
Methods & Materials: In the present systematic review, all the relevant
qualitative papers published from 2010 to October 2018, were examined. A
literature search was done in databases SID, Iran Medex, Iran doc, Magiran,
Science Direct, Medline/PubMed, Web of Science, Scopus, ProQuest, Google
Scholar, Cochran Library with keywords patient safety, surgical procedures,
operative, checklist, World Health Organization, implementation science,
qualitative research and their equivalent terms in Persian. Inclusion criteria
were articles written in English or Persian, qualitative studies and relevant to
the objectives of the study. Exclusion criteria were review articles, posters,
presentations, letters to editor and quantitative studies.
Results: The findings of the review of 14 qualitative studies showed that the
most important challenges in effective implementation of the surgical safety
checklist were unpredictable priorities, lack of collaboration and coordination
of the surgical team members, mismatch between the checklist and hospital
setting, lack of patient’s cooperation and lack of a planned approach towards
implementing the checklist. The strategies to improve the implementation of
the surgical safety checklist included checklist localization, improving the
collaboration and coordination of all the team members, training and
practicing, patient participation, and active organizational leadership.
Conclusion: The introduction of the surgical safety checklist to the health care
setting is a permanent challenge and requires ongoing evaluations and its
integration into the workflow in the hospital, active and effective leadership,
explanation of why and how to use it by managers and receiving support from the
organization. Continuous education, performance evaluation and the participation
of all the surgical team members in the implementation of the checklist are key
factors for effective implementation of the surgical safety checklist.
