سنخ‌شناسي رابطه پزشكان داراي پرونده قصور پزشكي با بيماران: يك مشاهده همراه با مشاركت

The typology of the relationship between physicians who had history of medical malpractice lawsuit and patient: a participant observation

چگونگيِ رابطه پزشك-بيمار مي‌تواند از عوامل تعيين‌كنندة شكايت بيمار از پزشك به علت قصور پزشكي باشد. اين مطالعه با هدف سنخ‌شناسي رابطه پزشكان داراي پرونده شكايت قصور پزشكي با بيمار انجام شد. در اين مطالعة كيفي داده‌ها به روش مشاهدة همراه با مشاركت جمع‌آوري شد و محقق به‌عنوان بيمار به مطب پزشكاني كه داراي پرونده‌هاي متعدد شكايت از قصور پزشكي در سال 1394 بودند مراجعه و حاصل مشاهدات را بعد از خروج از مطب يادداشت و تحليل كرد. سه سنخ مطب و الگوي رابطه پزشك-بيمار قابل شناسايي بود. اول، مطب همچون يك بنگاه تجاري كوچك با رابطه پزشك/كاسب-بيمار/مشتري؛ دوم، مطب همچون يك قصر كوچك با رابطه پزشك/سرمايه‌دار-بيمار/هم‌طبقه؛ سوم، مطب همچون يك فضاي اداري و ايدئولوژيك با رابطه پزشك/مدير- بيمار/ارباب رجوع. رابطه پزشكان داراي پرونده قصور پزشكي با بيماران رابطه‌اي متكي به قدرت و منفعت است كه مي‌تواند فراهم‌كنندۀ زمينه شكايت بيمار از پزشك باشد.

Doctor-patient relationship model is one of the determinant factors that can prompt patients to file a lawsuit against doctor. The study aimed to determine the relationship models between physicians who had history of medical malpractice lawsuit and patients. In this qualitative study data collection conducted by participant observation method, and the researcher referred to the doctor as a patient. She noted and analyzed the results of her observations after leaving the office. Three categories of doctor-patient relationship were identified: first, medical office as a small business enterprise with physician/tradesman-patient/customer relationship; second, medical office as a small palace with physician/Bourgeois man- high class patient relationship; third, medical office as a government office whit physician/manager-patient/client relationship. The findings of this study showed that the relationship between physicians who had history of medical malpractice lawsuit and patients is based on financial interests and authority that can leads patient to make a medical malpractice lawsuit against doctor.
