چكيده فارسي :
مقدمه: بهبود شرايط بهداشتي و تغيير وضعيت اقتصادي اجتماعي در ايران، به همراه شهرنشيني برنامه ريزي نشده، منجر به تغيير الگوي بيماري از بيماري هاي واگيردار به بيماري هاي غيرواگير شده است. هدف از انجام اين مطالعه طراحي الگوي پياده سازي مديريت يكپارچه پيشگيري و كنترل بيماري هاي غيرواگير در ايران بود.
مواد و روش كار: يك مطالعه كيفي 4 مرحله اي شامل تحليل اسناد(1)، مرور مقالات و گزارش هاي علمي(2)، مصاحبه هاي عميق فردي و بحث هاي گروهي متمركز(3) و تأييد كميته راهبردي (4) جهت تحليل وضعيت و شناخت شكاف خدمات ، تعيين جهت گيري شامل دورنماي 5 سال آينده، اهداف راهبردي در انتهاي 5 سال و راهبردهاي اصلي و مداخلات و نظام استقرار و الزامات كنترل بيماريهاي غيرواگير انجام شد. داده ها با استفاده از روش تحليل محتوا تحليل گرديد.
يافته ها: پس از شناخت شكاف خدمات در حوزه هاي كلان و عملياتي، اهداف راهبردي در دو گروه مربوط به عوامل رفتاري و عوامل خطر زيستي تدوين شدند. سه راهبرد اصلي پيشگيري و كنترل بيماريهاي غيرواگير در ايران شامل ارتقاي سواد سلامت گروه هاي جمعيتي، بهره مندي مردم از خدمات مراقبت هاي اوليه و نهادينه سازي همكاري فرابخشي براي كاهش عوامل خطر مرتبط با تغذيه، دخانيات، الكل و كاهش تحرك فيزيكي معين است. از الزامات برنامه طراحي شده تمركز ساختارهاي موازي و پراكنده در مديريت بيماري هاي غيرواگير وزارت بهداشت، آموزش نيروي انساني، تدوين بسته هاي خدمتي ادغام يافته، تعيين اولويت هاي تحقيقاتي، تبيين نقش هر يك از دستگاه ها، آگاهي كليه مديران دولتي و خصوصي و مشاركت نهادهاي مردمي است.
نتيجه گيري: جهت اجراي اين برنامه علاوه بر رعايت الزامات استقرار نيازمند اتخاذ تدابير ديگري مانند ادغام در برنامه بهداشت كشور، تصويب شوراي عالي سلامت و امنيت غذايي كشور و تدوين برنامه جامع سلامت استان ها مبتني بر وضعيت بومي مطابق اين برنامه است.

Objective (s): Improving health conditions and changing socioeconomic status in Iran, along with unplanned urbanization, has led to change in the pattern of communicable to non-communicable diseases. The aim of this study was to design a plan for implementation of integrated prevention and control of non-communicable diseases in Iran.
Methods: A 4-step qualitative study including document analysis, review of articles and scientific reports, in-depth individual interviews and focused group discussions and approval by the String Committee for Status Analysis and Service Gap Recognition was conducted. Then 5-year vision, strategic goals in the next 5 years and the main strategies and interventions and required re-organization were provided. Data were analyzed using content analysis method.
Results: Strategic goals were formulated in two groups related to behavioral and biological risk factors. Three main strategies for prevention and control of non-communicable diseases included promotion of health literacy, utilization of primary care services, and stabilization of intersectoral cooperation to reduce risk factors related to nutrition, tobacco, alcohol, and physical activity. Designed plan requirements included centralization of parallel and dispersed structures in non-communicable diseases management of the Ministry of Health, training human resources, developing integrated service packages, setting research priorities, explaining the role of each organization, increasing awareness of all public and private managers, and non-governmental organizations participation.
Conclusion: In order to implement this program, in addition to the consideration of the deployment requirements, other measures such as integration into the country's health program, the approval in the Supreme Council of Health and Food Security of the country, and the development of a comprehensive provincial health plan based on this program are required.
