نيازسنجي آموزشي پرستاران اورژانس در رابطه با بكارگيري دستگاه تهويه مكانيكي

Training Needs Assessment of Emergency Nurses Regarding the use of Mechanical Ventilation

زمينه و هدف نحوه مواجهه پرستاران اورژانس با بيماران تحت تهويه مكانيكي متغير بوده و دانش پرستاران در اين زمينه محدود مي باشد. لذا پژوهش حاضر با هدف تعيين نيازهاي آموزشي پرستاران بخش اورژانس در رابطه با بكارگيري دستگاه تهويه مكانيكي انجام شد. روش در اين مطالعه كه به صورت توصيفي بود، تعداد 98 نفر از پرستاران شاغل در بخش اورژانس بيمارستان هاي منتخب دانشگاه ايران پرسشنامه را تكميل نمودند. به منظور نيازسنجي پرستاران، از پرسشنامه پژوهشگرساخته مشتمل بر اطلاعات جمعيت شناختي، سنجش دانش درقالب سناريو، سنجش نگرش و برگه مشاهده مهارت كه پس از تاييد روايي وپايايي، استفاده شد. تجزيه و تحليل داده ها با استفاده از نرم افزار SPSS نسخه 22 و آزمون هاي (پارامتريك و غير پارامتريك) انجام شد. يافته ها اكثريت پرستاران شركت كننده، 7/86 درصد زن بودند و در گروه سني 25 تا 35 سال قرار داشتند. 5/74 درصد پرستاران بخش اورژانس سابقه كمتر از يك سال در بخش ويژه داشتند و 51 درصد دوره پرستاري اورژانس و 4/72 درصد پرستاران دوره پرستاري ويژه را نگذرانده بودند. بيشترين نياز آموزشي در حيطه دانش، مربوط به دلايل اينتوباسيون بيمار؛ در حيطه مهارت، نحوه تنظيم پارامترهاي ونتيلاتور؛ و در حيطه نگرش، ترس در مراقبت از بيماران تحت ونتيلاتور بود. نتيجه گيري با توجه به نتايج نيازسنجي ضروري است كه در برنامه ريزي آموزشي براي پرستاران به مشكلات و نيازهاي آن ها توجه كافي مبذول شود. به منظور رفع نيازهاي حرفه اي، بايد دوره هاي آموزشي كوتاه مدت يك روزه برگزار و جنبه هاي كاربردي و مهارتي را در آموزش پرستاران مدنظر قرار دهند.

Background and aim The way that the nurses working in Emergency Departments (ED) encounter the patients under mechanical ventilator is different and their knowledge is limited in this field. Therefore the present study aims to determine the training needs of nurses related to the utilization of mechanical ventilators. Method In this descriptive study, 98 nurses working at emergency departments of top rated hospitals of Iran University of Medical Sciences filled the questionaire. This researcher designed questionaire included demographic data, evaluation of knowledge in the format of scenario, evaluation of attitude and a paper of skill checklist , were used after validity and reliability confirmation. Analysis of data were done by SPSS version 22 and parameteric and non-parameteric tests. Results Most of the nurses participanting in this study were female (86.7%) and in the age group of 25 to 35 years. Among them, 74.5% of ED nurses had work experience less than 1 year in the Intencive Care Unit (ICU), and 51% hadn’t passed ED nusing course. The most training need in the area of knowledge included patients intubation; in the area of skill included setting the ventilator’s parameters and in the area of attitude included fear of taking care of patients under ventilator. Conclusion According to the results of need assessment, it is mandetory to pay attention to the problems and demands of nursing in their educational programming. In order to meet profesional needs, short daily educational courses should be held and practical and skill aspects should be considered in training nurses.
