Explaining the Performance-based Payment Model with an Educational Approach in Iranian Educational and Medical Centers and its Comparison with the Existing Models in the Health Care System of the Countries Implementing this scheme

زمينه و هدف: هدف از مطالعه حاضر تبيين و مقايسه تحليلي مدل پرداخت مبتني بر عملكرد با رويكرد آموزشي در مراكز آموزشي و درماني كشور است. روش بررسي: اين پژوهش از لحاظ هدف، توسعه اي -كاربردي و به لحاظ روش شناسي دو روش تحليل ساختار - درون مايه (Structure -Thematic) و روش اسنادي (Documentary) است. روش نمونه گيري مورد استفاده روش نمونه گيري هدفمند و تدريجي بود كه اشباع نظري نمونه گيري در 10نفر احراز شد. براي جمع آوري اطلاعات در خصوص مولفه هاي سيستم پرداخت فعلي از روش مصاحبه نيمه ساختار يافته استفاده شد. همچنين براي دستيابي به اعتماد از دو ابزار بازرسي خارجي و اعتبار بيروني استفاده شد. براي تحليل داده هاي كيفي، كد گذاري و تدوين مدل موجود از نرم افزار Nvivo 12 استفاده شد. يافته­ ها: يافته نشان مي‌دهد از بين 22 مولفه تاثير گذار در فرمول محاسبه پرداخت مبتني بر عملكرد در مراكز آموزشي و درماني كشور در عمل تنها 7 متغير اصلي و 5 مؤلفه فرعي دخيل بوده و متغير هاي تاثير گذار ديگر مانند مقوله آموزش در اين سيستم مالي لحاظ نشده است. نتيجه­ گيري: در مدل پرداخت مبتني بر عملكرد در ايران عليرغم اختصاص بيشترين زمان كاري توسط پزشكان و كاركنان در مراكز آموزشي و درماني به مقوله آموزش دانشجويان، تفاوت چنداني در شيوه پرداخت مبتني بر عملكرد اين مراكز با مراكزي كه صرفا درماني هستند، وجود ندارد.

Background and Purpose: This study aims to explain and analytically compare the performance-based payment models with an educational approach in the educational and medical centers of Iran. Analysis Methods: This is and applied- developmental research in terms of purpose and the two methods of structure-thematic and the documentary analysis methods have been used. We have used purposive and sequential sampling methods and the theoretical saturation of sampling was achieved in 10 people. The semi-structured interviews were used to collect information about the components of the current payment system. The external inspection and external validity were used to reach the reliability, as well. To analyze qualitative data, the existing model was coded and compiled using Nvivo 12 software. Findings: The findings indicate that among the 22 most influential components in the performance-based payment calculation formula in the Iranian educational and medical centers, only 7 main variables and 5 subcomponents were practically involved and other influential variables such as the category of education is not included in this financial system. Conclusion: In the performance-based payment model in Iran, despite physicians and staff in the educational and medical centers allocate most of their working time to the student education, there is not a large difference in the payment method based on the performance of these centers with the centers that are purely medical.
