چالش هاى برون سپارى ارائۀ خدمات سلامت به بخش خصوصي از ديدگاه كاركنان دانشگاه علوم پزشكي مشهد

The Challenges of Outsourcing Health Services to the Private Sector from the Viewpoint of Employees of Mashhad University of Medical Sciences: A Qualitative Study

زمينه و هدف يكي از راهبردهاي ساختاربندي مجدد در نظام سلامت مشاركت بخش خصوصي و تكيه بر بازار است. عدم توجه به موانع و چالش هاي برون سپاري خدمات سلامت به بخش خصوصي ممكن است به شكست طرح منجر شود و صدمات جبران ناپذيري بر بدنه نظام سلامت وارد آورد. اين مطالعه با هدف شناسايي چالش هاي برون سپاري خدمات سلامت به بخش خصوصي نگاشته شده است. روش بررسي مطالعه حاضر از نوع كيفي با رويكرد تحليل محتوا است كه به صورت مقطعي در نيمه دوم سال 1395 انجام گرفت. نمونه گيري به صورت هدف مند بوده و تا اشباع داده ها ادامه يافت. نمونه پژوهش شامل 16 نفر از صاحب نظران در حوزه برون سپاري و مديران و كارشناسان بهبود كيفيت بيمارستان بودند. جمع آوري داده ها با مصاحبه عميق و تحليل داده ها با روش تحليل محتوا و با درنظرگرفتن معيارهاي موثق بودن، معتبربودن و قابليت تثبيت داده ها انجام گرفت. يافته ها يافته هاي پژوهش شامل هفت چالش اصلي بود: مسائل اقتصادي (4 زير موضوع)، مشكلات ساختاري (12 زيرموضوع)، چالش هاي نيروي انساني (7 زيرموضوع)، مسئوليت و پاسخ گويي (4 زيرموضوع)، چالش هاي برنامه ريزي (7 زيرموضوع)، كارايي (5 زيرموضوع) و شركت هاي خصوصي (7 زيرموضوع). نتيجه گيري برون سپاري خدمات سلامت به بخش خصوصي با چالش هاي زيادي روبه رو است و رفع آنها مي تواند گام بزرگي در ارتقاي كيفيت خدمات سلامت باشد. مشكلات ساختاري از موضوع هاي اصلي و پردامنه مطرح شده از طرف مصاحبه شوندگان بود. پيشنهاد مي شود، مديران قبل از اجراي طرح هاي برون سپاري، تهميدات لازم به منظور دوري هرچه بيشتر از اين چالش ها را در نظر بگيرند

Background and Purpose: One of the strategies of the the health system is (participation of) private sector and relying on the market. Lack of attention to the obstacles and challenges of Outsourcing Health Services to the Private Sector may lead to failure and irreparable damage to the health system. The present study aimed at identifying the challenges of Outsourcing Health Services to the Private Sector. Materials and Methods: This study was done in the form of qualitative research using a content analysis method conducted in a cross-sectional manner in late 2016. To this end, sampling was a purposive type and it continued until the data saturation was fulfilled. The participants consisted of 16 experts, managers and Quality improvement experts of hospitals Data collection was conducted through in-depth interviews. Content analysis approach was used to data analyzing with regard to validity, reliability, and consolidation capabilities of data. Findings: Findings were included seven main challenges: Economic issues (4 sub-theme), Structural problems (12 sub-theme), Manpower challenges (7 sub-theme), Responsibility and accountability (4 sub-theme), planning challenges (7 sub-theme), Efficiency (5 sub-theme) and private companies (7 sub-theme). Conclusions: There are many challenges in Outsourcing Health Services to the Private Sector and Solv them can be a step in achieving improve the quality of health services. Structural problems were the major issues raised by interviewees. It is suggested, administrators before Outsourcing, in order to avoid any more of these challenges
