Design and Evaluation of an Anesthesia Decision Support Mobile Application

بيهوشي يكي از فرآيندهايي است كه خطاي دارويي در آن رايج است. يكي از پرتكرارترين خطاهاي دارويي در بيهوشي، خطا در محاسبه دوز داروها مي‌باشد. سيستم‌هاي پشتيبان تصميم يكي از راه‌حل‌هاي كاهش اين خطاها مي‌باشند. هدف اين مطالعه طراحي و ارزيابي يك نرم‌افزار تلفن همراه پشتيبان تصميم بيهوشي جهت كمك به متخصصان بيهوشي براي تجويز صحيح و به موقع داروها مي‌باشد. روش اين مطالعه مقطعي-كاربردي در سه مرحله نيازسنجي، طراحي و ارزيابي صورت گرفت. نيازسنجي اين نرم‌افزار با استفاده از مصاحبه نيمه ساختاريافته با پزشكان انجام شد. بعد از رسم طرح اوليه، نرم‌افزار تلفن همراه براي سيستم عامل اندرويد طراحي شد و در انتها در دو شاخص دقت و سرعت محاسبه، ارزيابي شد. نتايج خروجي مصاحبه در سه دسته ديدگاه پزشكان در مورد ضرورت ساخت نرم‌افزار، امكانات و قابليت‌هاي مورد انتظار براي نرم‌افزار و محتواي اطلاعاتي نرم‌افزار طبقه‌بندي شد. نتايج ارزيابي اين نرم‌افزار نشان داد در تمامي گروه‌ها به جزء گروه متخصص، زمان محاسبه توسط پزشكان بيشتر از نرم‌افزار بود. همچنين بيشترين پاسخ صحيح توسط گروه كاربر نرم‌افزار و سپس گروه دستيار سال چهارم بود. نتيجه‌گيري وجود يك نرم‌افزار پشتيبان تصميم بيهوشي براي پزشكان به طور قابل‌توجهي در افزايش دقت و سرعت محاسبات دارويي پزشكان تاثيرگذار مي‌باشد. تا‌كنون در ايران هيچ نرم‌افزار بيهوشي بر اساس دستورالعمل‌هاي معتبر جهاني ساخته نشده است. بنابراين نتايج اين مطالعه مي‌تواند مورد توجه ارايه‌دهندگان خدمات سلامت قرار گيرد

Anesthesia is one of the processes in which medication error is common. One of the most frequent medication errors in anesthesia is the error in calculating the dose of drugs. Decision support systems are one of the available solutions to reduce these errors. The purpose of this study was to design and evaluate an anesthesia decision support mobile application to assist anesthesiologists to choose the right and timely prescription of drugs. Method This practical cross-sectional study was conducted in three stages including needs assessment, design, and evaluation. The needs assessment of this mobile app was performed using semi-structured interviews with physicians. After drawing the initial design, the mobile app was developed for the Android operating system, and in the end, it was evaluated based on accuracy and speed of calculation indicators. Results The output of the interviews was classified into three categories including physicians’ views on the need to develop the app, the expected facilities and capabilities of the app, and the information content of the app. The evaluation results of this app showed that in all groups, except the expert group, the calculation time required by physicians was longer than the one by the mobile app. Moreover, the highest number of correct answers was given by the app user group and then the fourth-year resident group. Conclusion An anesthesia decision support mobile app for physicians can significantly increase the accuracy and speed of physicians’ medication calculations. So far, no anesthesia mobile app has been developed in Iran according to valid international guidelines. Therefore, the results of this study can be taken into consideration by health service providers.
