تأمل در سرشت پزشكي: ضرورت‌ها، رويكردها و چالش‌ها

On the Nature of Medicine: Necessities, Approaches, and Challenges

با گذر از نيمه قرن بيستم ميلادي وقايعي رخ داد و پديده‌هايي برآمد كه در مجموع آنها را «بحران پزشكي» خواندند. اين بحران سبب شد فلسفه كه از نيمۀ قرن نوزدهم از پزشكي منتزع شده بود مجالي براي حضور مجدد بيابد. فيلسوفان پزشكي، بحران را به فربه‌شدن وجوه علمي و تكنيكي و نحيف شدن جنبه‌هاي انساني طبابت نسبت دادند. از همين رو، تأمل در باب اينكه پزشكي چيست و چگونه بايد ميان وجوه علمي-تكنيكي و انساني آن توازن برقرار كرد به يكي از مسائل محوري فلسفۀ پزشكي بدل شد. سرشت پزشكي معمولاً در غالب دوگانۀ علم-هنر صورت‌بندي مي‌شود. در اين مقاله ابتدا اين صورت‌بندي را تشريح مي‌كنم و سپس نابسندگي آن را نشان خواهم داد. در پايان الگويي پيشنهاد مي‌دهم كه بتوان سرشت پزشكي را بيرون از دايرۀ دوگانۀ علم/هنر فهم كرد. در اين الگو پزشكي پركتيسي است علمي، مبتني بر قضاوت، گفتگو، مفاهمه و استوار بر ساماني اجتماعي. اين الگو با چالش‌هاي غايت‌شناختي، معرفت‌شناختي، علم‌شناختي، سياست‌گذارانه و حرفه‌مندي روبروست كه نيازمند تأملات جدي و پيوسته در سرشت پزشكي است.

After the middle of the twentieth century, symptoms have appeared gradually which were collectively called the “crisis of medicine”. This crisis gave philosophy, which had been dissected from medicine since the mid-nineteenth century, an opportunity to rejoin. Medical philosophers attributed the crisis to the inflation of the scientific and technical aspects and, consequently, to the weakening of the human aspects of medicine. Therefore, reflection on the nature of medicine became one of the central issues of philosophy in medicine. The nature of medicine has been framed based on the science-art dichotomy. Because both science and art do not have net meaning and many conceptions have been developed, in the first part of the paper I will try to explain and clarify the different conceptions of medicine as science and medicine as art. My main thesis in this paper is that the science-art dichotomy is not an appropriate framework for understanding the nature of medicine. Finally, I will propose a model to put the science-art dichotomy aside.
