معرفي يك مورد حاملگي موفقيت‌آميز در بيمار مبتلا به آرتريت تاكاياسو

SUCCESSFUL PREGNANCY IN A PATIENT WITH TAKAYASU’S ARTERITIS: A CASE REPORT

آرتريت تاكاياسو يا به‌اصطلاح بيماري بدون نبض يك بيماري بسيار نادر در جوانان است كه علائم اوليه آن در سن 5 تا 40 سالگي ايجاد مي‌شود. اگرچه ممكن است هر دو جنس تحت تأثير قرار بگيرند، 80-90 درصد بيماران زن هستند. اين بيماري، آرتريت التهابي مزمني است كه عروق بزرگ را درگير مي‌كند. اين شكل آرتريت مي‌تواند با آنژيوگرافي غيرطبيعي بخش فوقاني آئورت و شاخه‌هاي اصلي آن همراه باشد كه سبب اختلال در خون‌رساني عروق اندام فوقاني گردد. حاملگي همراه با سابقه آرتريت تاكاياسو جز حاملگي‌هاي پرخطر محسوب مي‌شود و بسته به پيشرفت بيماري و ميزان آسيب به عروق، احتمال خطر افزايش مي‌يابد. هدف از اين بررسي، گزارش يك مورد حاملگي موفقيت‌آميز و بدون عارضه در بيمار مبتلا به آرتريت تاكاياسو است. معرفي مورد: بيمار خانم 36 ساله حاملگي سوم صاحب دو فرزند سالم با سن حاملگي 38 هفته كه با دردهاي زايماني مراجعه كرده بود. در سابقه آرتريت تاكاياسواز 8 سال قبل داشته كه در بارداري فعلي هيچ‌گونه پيگيري انجام نداده و درماني دريافت نكرده بود. اولين مراجعه وي در 38 هفتگي به علت شروع دردهاي زايماني بود. براي بيمار بررسي‌ها و اقدامات اورژانسي انجام شد و درنهايت به دليل ارست ديلاتاسيون سزارين اورژانسي انجام گرفت كه حاصل اين بارداري نوزاد پسر سالم با اپگار 9-10 و وزن 4300 گرم بود.

Takayasu arteritis, a so-called pulseless disease, is a very rare disease in young people. The first symptoms appear at the age of 5 to 40 years. Although both sexes may be affected, 80-90% of patients are female. It is a chronic inflammatory arthritis that affects the large arteries. This form of arthritis can be associated with abnormal angiography of the upper aorta and its main branches, which disrupts blood flow to the arteries of the upper extremities. Pregnancy with a history of Takayasu arteritis is considered a high-risk pregnancy and the risk increases depending on the progression of the disease and the extent of vascular damage. The purpose of this article is to report the case of a successful and uncomplicated pregnancy in a patient with Takayasu arteritis. A 36-year-old woman in her third pregnancy with two healthy children, with a gestational age of 38 weeks was referred to us with labor pains. The patient had a 8-year history of Takayasu's arteritis. She did not follow-up and received no treatment during her current pregnancy. Her first visit in week 38 was due to the onset of labor pains. Emergency evaluation and procedures were performed. Finally, due to arrest dilatation, an emergency cesarean section was performed. She delivered a healthy boy weighing 4300 g with an Apgar score of 9-10.
