ارزيابي موانع و تسهيل‌گرهاي مؤثر بر پياده‌سازي پرونده الكترونيك سلامت در ايران

Evaluation of Barriers and Facilitators Affecting the Implementation of Electronic Health Records in Iran

مقدمه: با وجود توسعه فناوري اطلاعات در حيطه سلامت، هنوز هم فرايند به كار گيري پرونده الكترونيك سلامت، دشوار به نظر مي رسد. بنابراين، شناسايي موانع اجرايي، به حذف مشكلات و تعيين راهكارهاي مؤثر بر پياده سازي پرونده الكترونيك سلامت در ايران كمك خواهد كرد. مواد و رو ش ها: مطالعه حاضر از نوع مروري بوده و جامعه پژوهش شامل مستندات مرتبط با موانع و تسهيل گرهاي مؤثر بر پياده سازي پرونده الكترونيك سلامت بود. جستجو در پايگاه هاي اطلاعاتي Magiran ،Iran Medex ،SID و موتور جستجوگرGoogle Scholar انجام شد. از ميان 152 منبع بازيابي شده، تنها 14 منبع بررسي شدند. براي همين منظور، پيش نويس فرم گردآوري داده در نرم افزار اكسل طراحي و پس از دريافت نظرات متخصصان و انجام اصلاحات لازم روايي فرم نهايي تائيد شد. سپس، منابع منتخب براساس اهداف مطالعه در فرم گردآوري داده وارد شده و تحليل روايتي انجام شد. يافته ها: مهم ترين موانع اجرايي براي پياده سازي EHR، محدوديت هاي نگرشي و رفتاري افراد و تغييرات سريع سازماني بودند. بنابراين، براي تضمين نگرش مثبت ارائه دهندگان مراقبت سلامت نسبت به اين موضوع، توجه به آموزش و مشاركت كاربران در مراحل فرايند پياده سازي ضروري است. اگر كاربران سيستم، از اهداف، مزايا و تاثير مثبت EHR آگاهي داشته و استانداردهاي مربوط به حفظ محرمانگي، امنيت داده و قابليت تبادل پيام را درنظرگيرند، مقاومت آن ها نسبت به تغييرات كاهش يافته، و آمادگي براي پذيرش اين تغيير افزايش مي يابد. بحث و نتيجه گيري: با توجه به يافته هاي پژوهش حاضر به نظر مي رسد كه به كارگيري افراد با تجربه و متخصص، ارائه آموزش هاي لازم و بسترسازي فني جهت پياده سازي پرونده الكترونيك سلامت، همراه با اختصاص منابع مورد نياز، راه را براي به كارگيري اين فن آوري هموار مي سازد.

Introduction: Despite the development of information technology in the field of health, the process of creating and using electronic health records is still difficult. Therefore, identifying the implementation barriers of this system contribute to eliminate them and adopt effective implementation strategies. Methods and Materials: The present study is a review article and the research population included documents related to barriers and facilitators affecting the implementation of electronic health records. Databases were searched in SID, Iran Medex, Magiran and Google Scholar search engine. Only 14 sources were reviewed among the 152 recovered sources. For this purpose, the draft data collection form was designed in Excel software. The validity of the final form was approved after receiving the opinions of experts and making the necessary corrections. Then, the selected sources were entered based on the objectives of the study in the data collection form. After that, the narrative analysis was performed. Results: The most important implementation barriers in performing the EHR are the attitudinal and behavioral limitations of individuals and organizational changes. Thus, paying attention to education and the health care providers participation in implementing is essential to ensure that they have the right attitude. If system users know about the features, objectives, benefits and positive impact of the EHR on their work and they consider the standards related to data confidentiality as well as the ability to exchange messages, their resistance to the changes will decrease and their interest and readiness to accept the EHR increases. Discussion and Conclusion: Findings indicate that employing a team of experts in different areas, implementing essential education technical provision for performing EHR along with allocating financial resources can help overcome barriers to the adoption of EHR.
