چكيده فارسي :
امروزه بررسي شاخص هاي حركتي دهان در شيرخواران، به تيم درمان كمك مي‌كند تا شيرخواراني كه در اين شاخصها دچار تاخير هستند را زودتر شناسايي كنند و از ترتيب ظهور اين شاخصها در شيرخواران طبيعي، به عنوان معياري براي درمان شيرخواران با تاخير حركتي استفاده كنند. هدف از مطالعه حاضر، تعيين زمان ظهور شاخصهاي حركتي باز كردن دهان در هنگام چسبيدن قاشق به لب ها، حركــت آرام زبان به جلو و عقب در هنگام ورود غذا به دهان و حركت به طرفيــــــن بردن زبــــــــــان و فك در نوزادان طبيعي ايراني 4 تا 9 ماهه شهر شيراز است.
روش
در اين مطالعه مقطعي، در سال 1397 ، با استفاده از نمونه‌گيري خوشه‌اي،250 شيرخوار 9-4 ماهه طبيعي، از 7 مركز بهداشت انتخاب شدند. پس از تاييد رشد طبيعي اين شيرخواران با استفاده از(ASQ) The Ages & Stages Questionnaires يك كارشناس گفتاردرماني در محيط مركز بهداشت در ساعت تغذيه كودك، الگوهاي حركتي فك، زبان و لبها را در خوردن نيمه جامدها و جامدها در 6 گروه سني 4، 5، 6، 7، 8، و 9 ماهه ، با استفاده از چك ليست محقق ساز الگوهاي حركتي حركات فك و زبان، ارزيابي و تحليل كرد.
يافته ها
نسبت روايي محتوايي چك ليست ارزيابي حركات دهاني 0/99 بود. تمام شيرخواران 9-4 ماه، دو شاخص حركتي باز كردن دهان در هنگام چسبيدن قاشق به لب ها، حركــت آرام زبان به جلو وعقب در هنگام ورود غذا به دهان را نشان دادند. در بررسي حركت طرفي زبان، 13/6% كودكان 4 ماهه فقط حركت زبان به طرف راست را نشان دادند و 40/9% چرخش زبان به طرف راست و چپ را نشان دادند. و به ترتيب شاخص چرخش زبان به طرف راست و چپ در 5، 6، 7 ، 8 ، 9 ماهه‌ها، 73/5%، 59% ، 80/8%، 91/4% ، 93/3% بود. حركت به طرفيــــــن بردن زبــــــــــان در سن 5 ماهگي و حركات جانبي فك، در 8 ماهگي در شيرخواران ايراني ديده‌‌شد.
نتيجه گيري
به نظر مي‌رسد كه سن ظهور برخي از شاخصهاي مذكور در شيرخواران ايراني مشابه ساير نژادها و در برخي شاخصهاي ديگر، مانند حركات آرام جلو به عقب زبان هنگام ورود ماده غذايي به دهان، زودتر از نژادهاي ديگر ظهور پيدا كرده‌است. ضروري است كه مطالعات تعيين شاخصهاي حركتي، در جمعيتهاي مختلف بطور جداگانه صورت گيرد، تا اطلاعات دقيق‌تري جهت تعيين تاخيرهاي حركتي ناحيه دهان، در اختيار درمانگران قرار گيرد.
چكيده لاتين :
Introduction: Examination of oral motor indices in infants helps treatment team members identify infants who are delayed in these indices earlier and use the order of occurrence of these indices in normal infants as a criterion for treating infants with motor delay. Literature shows that oral movement indices are different in different races, cultures, and languages. The aim of this study was to determine the time of onset of movement indicators of opening the mouth when the spoon sticks to the lips, gently moving the tongue back and forth when food enters the mouth, and lateral movement of the tongue and jaw in Iranian normal infants in Shiraz.
Method: In the current study, in 2018, by using cluster sampling, 250 infants 4-9 months were selected from 7 public health centers in Shiraz. After conferring the participants ‘normal development, by using The Ages & Stages Questionnaires(ASQ), a speech therapist assessed and analyzed the lips, tongue, and jaws movements during feeding of semi-solid and solid food by using researcher developing Questionnaire in public health centers in 6 age groups include 4,5,6,7,8, and 9 months.
Results: The oral movements checklist content validity ratio (CVR) were 0/99. The oral movements, opening the mouth when the spoon sticks to the lips, moving the tongue gently forward and backward when food enters the mouth were observed in all participants 4-9 months. In evaluating lateral movement of the tongue, 13.6% of infants with 4 months showed only the tongue movement to the right, and 40.9% showed the tongue movement to the right and left. the tongue movement to the right and left in infants with 5, 6, 7, 8, and 9 months were observed in 73.5%, 59%, 80.8%, 91.4% and 93.3% respectively. Moving to the side of the tongue and lateral jaw movements are seen in Iranian infants at the age of 5 and 8 months, respectively.
Conclusion: According to the results, the age of onset of some of oral movements in Iranian infants is similar to other races and the age of onset of some of oral movements such as moving the tongue gently forward and backward when food enters the mouth is sooner than other races. It is necessary to conduct more extensive studies in different populations to provide exact indexes about oral movements to diagnose oral motor delays.
