الگوي اخلاق حرفهاي در نظام آموزش سلامت حوزه معاونت بهداشت دانشگاه‌هاي علوم پزشكي شمالغرب كشور

Model of Professional Ethics in the Health Education System of the Vice Chancellor for Health of the Northwestern University of Medical Sciences

پژوهش حاضر با رويكرد تركيبي آميخته اكتشافي متوالي كيفي و كمي در اخلاق حرفه‌اي نظام آموزش سلامت حوزه معاونت بهداشت دانشگاه‌هاي علوم پزشكي شمالغرب كشور انجام گرفت. در بخش كيفي تعداد 43 نفر از خبرگان شامل اساتيد آموزش بهداشت دانشگاه‌هاي علوم پزشكي شمالغرب كشور با روش نمونه گيري هدفمند و نمونه گيري نظري انتخاب شدند و داده‌هاي كيفي با استفاده از مصاحبه عميق در زمينه موضوع تا مرحله اشباع گردآوري شد پس از جمع آوري داده‌ها و تعيين مفاهيم و مقوله ها، اطلاعات در اختيار شركت كنندگان كليدي قرار گرفت و نظرخواهي شد و براي تاييد روايي بيروني مقايسه هايي با ادبيات موضوع انجام گرفت. كد گذاري هاي باز، كدگذاري محوري و تحليل كدگذاري گزينشي انجام شد . پس از استخراج مولفه‌هاي نهايي پرسشنامه 60 سوالي بر اساس مباني نظري و اطلاعات مصاحبه ها، تنظيم گرديد. پايايي پرسشنامه با ضريب آلفاي كرونباخ 92/0 مورد تاييد قرار گرفت. نمونه در بخش كمي بصورت تصادفي طبقه اي نسبتي از بين كليه بهورزان خانه‌هاي بهداشت و كارشناسان آموزش سلامت حوزه معاونت بهداشت دانشگاه‌هاي علوم پزشكي شمالغرب به تعداد 473 نفر انتخاب شد. پس از توزيع پرسشنامه، دادههاي استخراج شده با استفاده نرم افزارspss و ليزرل تحليل شد. نتايج حاصل از تحليل عاملي تاييدي و معادلات اندازه گيري براي هر يك از سازه ها بر اساس مقدار بارهاي عاملي به دست آمده معنادار بوده و مقدار بارعاملي همه شاخص ها بزرگ تر از 3/0 بدست آمد . مدل اندازه گيري برازش يافته از نيكويي برازش مناسبي برخوردار بود و مورد تاييد قرار گرفت. اعتبار الگو با تحليل عاملي تاييدي و ضريب بالاي آلفاي كرونباخ مورد تاييد قرار گرفت و در نهايت الگوي نهايي اخلاق حرفه اي در نظام آموزش سلامت حوزه معاونت بهداشت شمالغرب كشور با مولفه هاي وفاداري؛ مسئوليت پذيري؛ رقابت طلبي؛ صداقت؛ احترام به ديگران؛ احترام نسبت به ارزش‌ها و هنجارهاي اجتماعي ؛ عدالت و انصاف ؛ همدلي با ديگران ؛ نقدپذيري؛ قاطعيت؛ اعتماد؛ همكاري بين بخشي ؛ رعايت نظم؛ نگرش مثبت و تعهد ارائه گرديد. همچنين درجه اهميت هر يك از مولفه‌هاي اخلاق حرفهاي در نظام آموزش سلامت حوزه معاونت بهداشت دانشگاه‌هاي علوم پزشكي شمالغرب كشور تعيين گرديد.

The present study was conducted with the aim of designing a model of professional ethics in the health education system of the Vice Chancellor for Health of the Northwestern University of Medical Sciences. This combined study was conducted as a combination of qualitative and quantitative sequential exploratory mix. In the qualitative section, 43 experts including health education professors of northwestern medical universities were selected by purposive sampling and theoretical sampling and qualitative data were collected using in-depth interviews on the subject up to saturation stage after data collection. an‎d definition of concepts and categories, information was provided to key participants and opinion polls were conducted and comparisons with the literature were made to confirm the external validity. Open coding, axial coding and selective coding analysis were performed. After extracting the final components of the questionnaire, 60 questions were prepared based on the theoretical foundations and information of the interviews. The reliability of the questionnaire was confirmed by Cronbach's alpha coefficient of 0.92. In the quantitative section, 473 people were randomly selected from all health care providers and health education experts in the field of health of Northwestern University of Medical Sciences. After distributing the questionnaire, the extracted data were analyzed using SPSS and LISREL software. The results of confirmatory factor analysis and measurement equations for each of the structures were significant based on the amount of factor loads obtained and the amount of factor load of all indicators was greater than 0.3. The fitted measurement model had a good fit and was approved. The validity of the model was confirmed by confirmatory factor analysis and Cronbach's high alpha coefficient, and finally the final model of professional ethics in the health education system of the Northwest Health Department with loyalty components; responsibility; Competitiveness; Honesty; Respect for others; Respect for social values ​​and norms; Justice and fairness; Empathy with others; Liquidity; decisiveness; the trust ; Cross-sectoral cooperation; Observance of order; positive attitude; A commitment was made. Also, the degree of importance of each component of professional ethics in the health education system of the health department of the northwestern universities of medical sciences was determined.
