THE EFFECT OF DEXAMETHAZONE DEMINSTRA TION IN EPIDURAL SPACE ON BACKACHE REDUCTION

Local anesthesia (LA) is interesting in the field of anesthesia . According to the minor risk of the procedure in comparison with general anesthesia (GA) , One of the LA Method is epidural anesthesia . Advantage of this method is absence of headache . Severe haemodynamic changes which are important in cardiovasculars .However, backache is common complication of epidural anesthesia . This work was carried out prospectively in double blind condition on patients aged 30-50 years old who were candidate for hernioraphy operation . The patients had not a history of neither backache nor systemic disease. Patients were divided into two groups of 30 each. It was cared to keep unfeelingness agent and dexametazone (4mg) was injected during the standard epidural anesthetized procedure. The control group, a combination of unfeelingness agent and normal saline was injected. The severity of pain in patients was measured using visual analog scale (VAS) and data were analyzed using SPSS for windows . Average of pain intensity was significantly higher in control group than test group in both sexes: in men 3.8 ± 2.1 and 2.5 ± 2.24 and in women 5 ± 2.62 and 2.7 ± 2.44 , respectively . When patients were only one time anesthetized , again the severity of pain was higher in control comparing with test group, 3.9 ± 2.08 , and 2.3 ± 2.09 , respectively . This condition was seen in patients that were anaesthetized more as 7.7 ± 2.06 , and 5 ± 2.45 in control and test group , respectively. The severity of pain was under 3 units in 80 % of men and 73.3% of women. In remaining , 20% of men and 26.7% of women had between 3 and 7 units , in test group. While in control group, severity of pain was under 3 unites in 40 % of men and 20% of women . The severity of pain was higher in patients who underwent more than one insertion . Injecting of 4 mg dexamethazone help to reduce the pain of needle entry and inflammation . More study need to find exact relation between influence of steroids on pain during epidural anesthetized procedure .
