Accuracy of Ultrasound in the Diagnosis of Acute Appendicitis

Introduction: Acute appendicitis is the most common cause of acute abdomen. Graded compression ultrasonography is the least expensive and the least invasive method of diagnosing appendicitis. Furthermore, the aforementioned method has been reported to have an accuracy of 71 to 95 per cent and thus it is an accurate aid in establishing the diagnosis of appendicitis. This study was aimed to determine the accuracy of ultrasonography in the diagnosis of acute appendicitis in patients referred to Bessat Hospital in Sanandaj city (IRAN) between 2002 and 2003. Materials & Methods: Statistical population sample included all patients with acute appendicitis that were admitted to Bessat Hospital. This was a descriptive study. Clinical and ultrasound findings along with the results of the laparatomies in 150 patients were evaluated. Data was collected and analyzed using Spss Win.Software. Results: In this study, a total of 170 patients with clinically diagnosed or suspected appendicitis underwent an abdominal sonography examination performed by sonographists before operation. The sonographic findings were classified into three categories: appendicitis, other diseases, or normal screening. A total of 150 patients (88.2%) underwent operation, of which 141 (94%) proved to have appendicitis by the pathologic reports. A total of 28 negative appendectomies (16.8%) were prevented after sonographic examination. Abdominal sonography for detecting acute appendicitis had a sensitivity of 98.61%, a specifity of 66.66%, an accuracy of 89%, a positive predictive value of 94% and a negative predictive value of 90%. Conclusion: According to the results of our study patients with clinically diagnosed or suspected acute appendicitis should routinely undergo abdominal sonography examination performed by an experienced surgeon to decrease the negative appendectomy rates.
