A Survey on the factors Affecting Prehospital Delay in Patients with Acute Myocardial Infarction

To Evaluate factors affecting the time between symptoms onset and hospital admission in patients with acute myocardial infarction (AMI) , a detailed questionnaire was given to all those who were admitted with AMI , from December 1997 to September 1998 for the 120 patients (69 men , 51 women) the mean prehospital time was 26.2+34 hours (median 5 , range from 0.5 to 96) . In 71 patients , the prehospital time was less than or equal to 6 hours (early arrival) ; 49 patients were admitted at least 6 hours after the onset of the symptoms (late arrival) . Late arrival (more than 6 hours) was significatly more fuquent among patients experiencing slow symptom progression and psychological factors , having Low income and living far away from the hospital (P<0.05) . A significant difference was observed between the two groups . Also , stepwise multiple regression andlysis indicated that slow symptom progsession , low income , long distance fom the hospital and old age , have all led to prehospital delay . Thus , it is possible to edncate patients likely to experience udue prehospital delay during AMI to mininize the delay , thus minimizing the death rate , before referring to the hospital .
