ROLE OF H.PYLORI INFECTION IN ETIOLOGY OF GASTRIC ADENOCARCINOMA ACCORDING TO ITS ANATOMICAL LOCATION

Our aim was to evaluate the role of H.Pylori infection in etiology of gastric adenocarcinoma according to the anatomical location of the cancer . A case-control study was performed in Emam Khomeini hospital during the years 2000 - 2001. Thirty patients with cardia cancer in group 1 (case) and 45 subjects with non-cardia cancer in group 2 ( control) were included in the study. A detailed questionnaire about patients history and paraclinical results was filled.All patients underwent an UCIE and 2 biopsies from normal appearing mucosa of stomach were taken. The first biopsy was from fundal portion and the other was from antrum. Presence of H . Pylori was tested by RUT in the samples. At least 5 biopsies were taken from the tumoral lesion and these sample were evaluated pathologically. Serological assessment of H Pylori infection was performed in all patients . Mean age of patients was 66.7 years in group 1 and 63.2 years in group 2. Male to female ratio was 1.5 in both groups . Incidence of H. Pylori infection in tumoral lesion was significantly less than that in normal appearing mucosa .There was no significant difference between H. Pylori infection in patients of both groups (cardia and non cardia cancer , 70% versus 73.3%) .
