ASSESSMENT DEFERENT METHODE OF CHOLEDOCHOLITHIASIS TREATMENT AND REPORT OF 5 CASES TREATED BY LAPAROSCOPIC

Laparoscopic cholecystectomy is the technique of choice in symptomatic gallstone disease. Choledocholithiasis accompanies 10-15% of these patients and if diagnosed by preoperative investigation, it could be treated by ERCP and basket extraction. Sometimes diagnosis is proved during operation by cholangiography and based on diameter of the stone, it can be extracted from cystic duct or from the choledochotomy site by a basket. Laparoscopic treatment of choledocholithiasis requires much expertise and in advanced centers, it has been done successfully. Postoperative ERCP has been reported with good results in these patients as well. The aim of this study was to review details of all accepted procedures in treatment of choledocholithiasis and to introduce "one stage laparoscopic operation" as an acceptable method with the same clinical results but minimal morbidity and cost. The procedure has been performed at Imam Khomeini Hospital successfully for the first time and included choledochoduodenostomy, T-Tube insertion and trans-cystic duct stone extraction.
