Monitoring urinary tract infection in spinal cord injured patients remote to medical care: An Assessment of Urinary Nitrite dipstix self-testing

Assessment of the value of patient self-testing with Nitrite sticks in the rapid diagnosis of urinary tact infection (UTI) in spinal cord injured (SCI) patients.An outpatient population of forty consecutive traumatic SCI patients took part in the study (four quadraplegics and thirty-six paraplegics). Urine samples were collected to assess for nitrite, pyuria, bacturia, and they were also cultured. Histories were taken looking for symptoms of UTI. Statistical comparisons showed a significant relation between the results of Nitrite test (NT) and urine culture and bacteriuria, for the diagnosis of UTI (p=0.008 and 0.03, re-spectively). There were no significant correlations between the NT and urinary infec¬tion symptoms, and pyuria (p-value>0.05). Sensitivity and specificity of NT for urine culture and bacturia were 69ʹ/., 100ʹ/., 83 /., and 75ʹ/., respectively (Positive and negative predictive values of 100ʹ/., 38ʹ/., 83*/., and 75ʹ/., respectively). A positive NT is a useful guide for the diagnosis and treatment of UTI in SCI. How¬ever, this did not seem to be a reliable tool for screening.
