بررسي مشكلات همراه در كودكان مبتلا به فلج مغزي و ارتباط آنها با نوع و سطح عملكرد حركتي

Accompanying Problems in Children with Cerebral Palsy and their Relationship to type and Level of Motor Disability

زمينه و هدف: كودكان مبتلا به فلج مغزي داراي يكسري مشكلات همراه هستند كه اين مشكلات مي توانند نسبت به خود فلج مغزي اثر بيشتري روي كودك و خانواده اش داشته باشند. اين مطالعه به منظور بررسي شيوع 6 مشكل همراه شايع در كودكان فلج مغزي و تعيين ارتباط آنها با نوع و سطح عملكرد حركتي صورت گرفت. روش بررسي: در اين مطالعه 120 كودك 2 تا 14 ساله از 4 مركز توانبخشي شهر تهران انتخاب شدند. پس از كسب رضايت از والدين كودك، تاريخچه پزشكي كودك گرفته شد و مشكلات همراه استخراج گرديدند. سطح عملكرد حركتي درشت و همچنين نوع فلج مغزي مشخص گرديد. داده ها با استفاده از نرم افزار SPSS ويرايش 16 و آزمون هاي كولموگروف اسميرونوف و ضريب همبستگي اسپيرمن مورد تحليل قرار گرفتند. يافته ها: مشكلات گفتاري بيشترين و مشكلات شنوايي كمترين ميزان را به خود اختصاص دادند. نوع فلج مغزي با مشكلات ذهني، گفتاري و غذا خوردن و همچنين تعدد مشكلات همراه ارتباط معني داري داشت(05/0p<). بين سطح عملكرد حركتي با مشكلات ذهني، گفتاري، غذا خوردن، تشنج و تعدد مشكلات همراه ارتباط معني دار وجود داشت (05/0p<). نتيجه گيري: با توجه به شيوع زياد مشكلات همراه در كودكان فلج مغزي به خصوص موارد با درگيري حركتي شديد تر، تشخيص و درمان به موقع اين اختلالات مي تواند نقش به سزايي در ارتقاي كيفيت زندگي اين كودكان و همچنين خانواده آنها داشته باشد. م ع پ 1390؛‌10(1): 67-59

Background and Objective: There are some accompanying problems in children with cerebral palsy. These problems may affect the children and their family even more than cerebral palsy. The aim of this study was to investigate the rate of six common accompanying problems and their relationship to type and level of motor function. Subjects and Methods: In this study, 120 children aged 2 to 14 years old were selected from 4 rehabilitation centers in Tehran. At first consent of the childʹs mothers were obtained, and then childʹs medical history was gathered and accompanying problems were extinguished. The gross motor function level and the type of cerebral palsy were determined. The data was analyzed by SPSS-16 using kolmogrov smironov test and spearmanʹs rho correlation coefficient. Results:The speech disorder was the most and hearing problem was the least common among children. The type of cerebral palsy was associated significantly with intellectual disabilities, speech , feeding problems and number of problems (p<0.05).There was significant correlation between Gross motor function classification system and intellectual disabilities, speech, feeding problems , seizure and number of problems (p<0.05). Conclusion: According to high occurrence of accompanying problems in children with cerebral palsy especially more severe ones, early detection and treatment of these problems could enhance the quality of life of the children and their family.
