بررسي ميزان دقت دستگاه Spiral Tomography در برآورد موقعيت كانال دنداني تحتاني

A evaluation of Spiral Tomography in position of inferior dental canal

خلاصه: سابقه و هدف: در اعمال جراحي دقيق مانند جراحي‌هاي ايمپلنت دنداني برآورد موقعيت دقيق كانال دنداني تحتاني براي اجتناب از آسيب‌هاي وارده به اعصاب از اهميت ويژه‌اي برخوردار است. هدف از مطالعه حاضر؛ تعيين دقت دستگاه Spiral Tomography در تعيين موقعيت كانال دنداني تحتاني مي‌باشد. مواد و روش‌ها: در اين مطالعه توصيفي توموگرافي اسپيرال در چهار ناحيه متفاوت در سگمنت خلفي هر منديبل (ديستال فورامن منتال) توسط دستگاه Cranex tome multifunctional x-ray ساخت كارخانه Sordex فنلاند، انجام گرفت. سپس منديبل‌ها از اين 4 ناحيه توسط دريل مخصوص برش خوردند. اندازه‌گيري‌ها بر روي توموگرام (A، B، C و D) و منديبل خشك (Aʹ، Bʹ، Cʹ و Dʹ) براي تك‌تك نمونه‌ها صورت گرفت و ميزان تفاوت موقعيت خطاي كانال‌ها با آزمون PAIRED T-test مورد قضاوت آماري قرار گرفت. يافته‌ها: ميزان خطا mm 02/0 (33/0=SD) 0.33±0.02 ميلي‌متر بوده و 2/95% نمونه‌ها در محدوده قابل قبول mm 5/0± و 4/98% نمونه‌ها در محدوده قابل قبول mm 1± قرار گرفته‌اند. ميزان ضريب همبستگي در كل نمونه‌ها برابر 9989/0 شد كه در تمام نقاط نشانگر تكرارپذيري معنادار مي‌باشد. (001/0P < ) يعني به طور معني‌داري اين دو اندازه يكسان بوده است. نتيجه‌گيري: اسپيرال توموگرافي با دستگاه Cranex tome multifunctional unit اطلاعات دقيق و جزييات كافي در طرح‌ريزي درمان پيش از جراحي‌هاي ايمپلنت در ناحيه خلفي منديبل را فراهم مي‌كند.

Abstract: Background and Aim: For certain surgical procedures (e.g. placement of implants), an accurate localization of the mandibular canal is of utmost importance to avoid injuries to the neurovascular bundle. The aim of the present study was to evaluate, the accuracy of spiral tomography for the localization of the mandibular canal. Material and Methods: By means of the cranex tome multifunctional unit (orion corporation Sordex, Finland), tomographic slices were taken at 4 different locations in each posterior mandible segments (distal to the mental foramen). The mandibles were then sectioned at these 4 sites with a specialize designed drill. The following 4 measurements were preformed both on the tomograms (A, B, C and D) and the bone sections (Aʹ, Bʹ, Cʹ and Dʹ) at the 4 sites. Results: We found an average mean difference of 0.02 mm with a standard deviation of 0.33 mm. The reported data showed that %95.2 of cases are in ±0.5 mm and %98.4 of cases are in ±1 mm (overestimate and underestimate) Conclusion: Spiral tomography using the cranex tome multifunctional x-ray unit provides accurate information and sufficient detail for preoperative planning of implant in the posterior mandible.
