بررسي تاثير فيشورسيلانت انجام شده در برنامه جامعه نگر در پيشگيري از پوسيدگي سطح جونده دندان‌هاي مولر اول دايمي

Evaluation the Effect of the Fissure Sealant Performed in the Community Center in Occlusal Caries Prevention of the First Permanent Molars

سابقه و هدف: پيت و فيشور سيلانت به عنوان بخشي از برنامه جامعه نگر سلامت دهان و دندان با هدف كاهش پوسيدگي دندان‌هاي دايمي اجرا مي‌شود. اين مطالعه، با هدف بررسي تاثير فيشور سيلانت انجام شده در برنامه جامعه نگر در پيشگيري از پوسيدگي دندان‌هاي مولر اول دايمي دردانشكده دندانپزشكي زاهدان درسال1387انجام شد. مواد و روش ها: اين تحقيق به صورت يك مطالعه گذشته نگر تاريخي) historical cohort (انجام شد. برنامه فيشور سيلانت در مركز جامعه نگر دانشكده دندانپزشكي زاهدان از سال 1384 آغاز شده است. سه سال پس از انجام اين برنامه، وضعيت دنداني كودكاني كه در برنامه شركت كرده بودند (134 نفر گروه مورد ) با كودكاني از همان منطقه كه در برنامه شركت ننموده بودند ( 134 نفر گروه شاهد) مقايسه گرديد. وضعيت پوسيدگي دندان‌هاي دايمي در گروه مورد و شاهد با استفاده از آزمون كاي دو و T مورد تجزيه و تحليل قرار گرفت. يافته‌ها: متوسط سن كودكان در زمان قرار دادن سيلانت7/6 سال و بطور متوسط از زمان اجراي برنامه فيشور سيلانت آنها 38 ماه گذشته بود. ميزان پوسيدگي در گروه مورد 0/14% و در گروه شاهد 6/21% بود (001/0 > p). شانس بروز پوسيدگي براي افراد گروه شاهد نسبت به گروه مورد 5/1 برابر بود (RR). نتيجه‌گيري: برنامه پيت و فيشور سيلانت در پيشگيري از پوسيدگي سطوح اكلوزالي موفقيت‌ آميز بوده است اگر چه در مقايسه با ساير مطالعات موفقيت كمتري را نشان داد.

Background and Aim: Pit and fissure sealant, as a part of community oral health program, is performed with the purpose of caries prevention (reduction) in the permanent teeth. The purpose of this study was to evaluate the effect of the pit and fissure sealant performed in the community oral health program in caries prevention of the first permanent molars. Materials &Methods: This research was performed in the form of a historical cohort study. The fissure sealant program has been started in the Community Center of Zahedan dental school since 2005. The dental status of children who attended in the sealant program (case group: n=134) was compared with the children from the same area who were not exposed to the program (control group: n=134) 3 years after the implementation of the program. The findings of dental caries in the case and control groups were analyzed by ?2 and t-test. Results: The mean age of children at the time of sealant placement was 6.7 years and average of 38 months had passed after its placement. Dental caries in the case and control group was 14% and 21.6% respectively (p < 0.001). The risk of caries incidence in control group in comparison of the case group was 1.5 (RR). Conclusion: The pit and fissure sealant program was successful in caries prevention of occlusal surfaces, although it showed less effectiveness compared to other studies.
