Paracervical block is a relatively simple method that is used by obstetrician to provide analgesia during labour and D & C. Lidocaine as a local anesthetic agent is used in paracervical blockage that has some potential side effects. In this study from the point of Lidocain morbidity, Normal Saline was used for this purpose. A randomized comparison of Lidocaine and saline for cervical anesthesia was performed.
This is a double - blind randomized clinical trial. Number of patients at each group was 100 women.
The inclusion criteria were: uterine size < 14 weeks, patients who didnʹt have molar pregnancy, missed abortion, underlying painful condition. Subjects gave informed consent and were assigned randomly to receive Lidocain, %1 or Normal Saline.
Subjects rated their pain on visual analog scale in three occasions. Using the SPSS did age, education, parity, previous abortion was collected and statistical analysis. Chi-Square test, T-test and variance analysis was done. Age, education, parity, the history of previous abortion have not significance difference in two groups.
The mean ± SD of pain intensity after speculum insertion, during dilatation and during curettage in Lidocain, group were 22.1±23.6, 35.7±27.9, 52.15±27.2 and in Normal Saline group were 27.35±25.7, 41.15±32.3, 54.1+28.1, respectively. No significant difference was observed in pain rating reported in two groups (P>0.05). Complication rate was %7 in Lidocain group but Saline group has no side effect. Normal Saline is as effective as Leidocaine in paracervical anesthesia for dilatation & curettage.
