بررسي مقدماتي ميزان اثر بخشي مخاط چسب آفتوژل در درمان زخم‌هاي آفتي مينور مخاط دهان

A pilot study to evaluate the efficacy of mucoadhesive (Aphtogel) in the treatment of oral minor aphthous ulcers

چكيده مقدمه: اثر بخشي تريامسينولون در درمان علامتي آفت دهاني، در منابع پزشكي به عنوان بهترين درمان معرفي شده است. يكي از چالش‌هاي بزرگ دارو درماني در تمام دنيا، طريقه دارو رساني به بافت‌هاي هدف است و اين‌كه چگونه مي‌توان از كمترين ميزان دارو به بهترين نحو براي دسترسي به اندام آسيب ديده استفاده كرد. هدف از اين پژوهش مقدماتي، تمركز بر حداقل مقدار داروي لازم بر روي زخم آفتي مينور مخاط دهان و بررسي كارايي اين روش بود. مواد و روش‌ها: براي انجام اين كارآزمايي باليني مقدماتي (Pilot) دوسوكور، شكل جديد مخاط چسب با هدف دوگانه تمركز دارو و پانسمان زخم آفت، طراحي و ساخته شد. با اضافه نمودن داروي تريامسينولون به پايه مخاط چسب آزمايش‌هاي باليني بر روي 40 بيمار بدون بيماري سيستميك و يا سابقه مصرف دارو، انجام شد كه تنها 23 نفر از آنان در اين مطالعه تا پايان، همكاري نمودند و نتايج قابل استنادي از ايشان دريافت شد. 14 نفر دارو و 9 نفر دارونما دريافت كردند. درد بيماران با شاخص (Visual analogue scale) VAS و قطر آفت بر حسب ميلي‌متر اندازه‌گيري و نتايج توسط نرم‌افزار SPSS و آزمون آماري Mann-Whitney تجزيه و تحليل شد (05/0 = ?). يافته‌ها: در گروهي كه دارو دريافت كرده بودند، ابعاد ضايعه (026/0 = p value) و درد آفت (043/0 = p value)، در جلسه دوم به صورت معني‌دار كمتر از گروه دارونما بود. اما در جلسه سوم تفاوت معني‌داري مشاهده نشد (05/ > p value). نتيجه‌گيري: بر اساس نتايج اين پژوهش، اضافه نمودن تريامسينولون به پايه مخاط چسب، در كوتاه مدت اثر معني‌داري نسبت به دارونما بر روي اندازه و ميزان درد آفت دهان دارا است و مي‌توان از اين روش جهت كاهش درد و تسريع روند بهبودي آفت استفاده نمود. كليد واژه‌ها: استيوماتيت، آفت، روش دوسوكور، اثر دارونما.

Abstract Introduction: Triamcinolone has been introduced as the treatment of choice in symptomatic treatment of oral aphthous ulcers in the medical literature. One of the major challenges in drug therapy all over the world is how to deliver the drug to target tissues, and how to best deliver minimal amounts of the drug to the injured organ. The aim of this pilot study was to determine minimum dose of the required drug effective on the oral minor aphthous ulcers and evaluate its effectiveness. Materials and methods: In this double-blind pilot clinical trial, a new form of muccoadhesive with the dual aim of dressing of and concentration of the drug on the aphthous ulcers was designed and manufactured by adding triamcinolone to muccoadhesive base. The drug was applied to 40 patients with no systemic conditions or previous drug therapy. Only 23 of the patients cooperated during the whole trial and could provide documented data. Fourteen patients received the drug and 9 received a placebo. Pain severity was evaluated by VAS scale and maximum diameters of ulcers were measured in millimeter. Data were analyzed by SPSS software and Mann-Whitney test (?=0.05). Results: In patients who received the drug the diameter (p value = 0.026) and pain severity (p value = 0.043) were significantly less during the second appointment in comparison with the patients receiving placebo. However, during the third appointment there were no significant differences between the two groups (p value < 0.05). Conclusion: According to the results of this pilot clinical trial, addition of triamcinolone to the base of muccoadhesive has a significant effect on diminishing pain severity and diameter of oral ulcers in the short term in comparison to placebo and this approach can effectively be used to decrease pain severity accelerate ulcer healing.
