چكيده فارسي :
در دنياي كنوني هر موسسه‌اي كه از تكنولوژي اطلاعات و شبكه‌هاي ارتباطي بي‌بهره باشد، به تدريج از جريان ارتباطات جهاني دور خواهد ماند. از اين رو صنايع مختلف، ارتقا كيفيت و افزايش بهره‌وري خود در دنياي پر رقابت امروز را منوط به به‌كارگيري فناوري اطلاعات مي‌دانند. يقينا صنعت سلامت نيز علي‌رغم تمام فراز و فرودها از اين تحولات مصون نبوده است. نظام سلامت همواره به‌منظور ارتقا سطح سلامت و بهبود نتايج باليني و مالي خود در راستاي بهره‌مندي از آخرين دستاوردها و فناوري‌ها، گام‌هاي موثري برداشته است.
اعلاميه هزاره كه در پنجاه و پنجمين جلسه علني سازمان ملل متحد در سپتامبر 2000 توسط 189 كشور عضو مورد توافق قرار گرفت، فناوري اطلاعات و ارتباطات را ابزاري مهم براي دستيابي به جميع اهداف اين سازمان اعم از كاهش فقر، اصلاح ارايه خدمات آموزشي و بهداشتي، سهل‌الوصول‌تر ساختن خدمات دولتي و اعتلاي سطح بهداشت و سلامت جوامع، معرفي مي‌كند. لذا با توجه به اهميت مدارك پزشكي، به عنوان مهمترين و غني‌ترين منبع اطلاعاتي نظام سلامت، مي‌توان اذعان داشت كه يكي از ابعاد مهم كاربرد فناوري اطلاعات در نظام سلامت كه نقش موثري در افزايش اثربخشي خدمات سلامت ايفا مي‌نمايد، ايجاد مدارك پزشكي الكترونيك مي‌باشد.
ابزار پرونده‌هاي بهداشتي درماني الكترونيكي (EHR)، يك سيستم پرونده بهداشتي الكترونيكي است كه كادر بهداشتي و درماني را قادر به صرفه‌جويي در زمان و هزينه مي‌سازد و به وسيله سازماندهي پرونده‌هاي بيماران مراقبت از آنان را بهبود مي‌بخشد، همچنين فرآيند درمان و تشخيص را نيز آسان مي‌سازد. اين ابزار گزارشات كاغذي سنتي را با امكانات تكنولوژي جديد براي افزايش دقت و سرعت تكميل پرونده‌هاي بهداشتي درماني تركيب مي‌كند. در حالي‌كه استفاده از پرونده‌هاي كاغذي مشكلاتي را در پي خواهد داشت از قبيل اشتباهات وابسته به بدخواني، نسخه‌نويسي‌ها و اشتباه نوشتن كلمات، افزايش هزينه‌ها براي چاپ فرم‌ها، پوشه‌ها، كارت‌ها، خريد فروش صندلي و فايل‌ها و از همه مهم‌تر مكان بايگاني و نيروي انساني متخصص كه براي تربيت هر كدام از آنها هزينه‌هاي گزافي پرداخت مي‌شود. بنابراين براي رفع چنين مشكلاتي استفاده از EHR پيشنهاد مي‌شود.
البته با توجه به حساسيت اطلاعات، محرمانگي پرونده‌ها، امري مهم و ضروري است و در اين راستا كشورهاي مختلف دنيا با توجه به شرايط و موقعيتشان اصولي را تنظيم نموده‌اند. حفظ محرمانگي پرونده‌ها با توجه به قوانين و شرايط اجتماعي، اقتصادي، علمي، سياسي و فرهنگي جوامع مختلف، متفاوت مي‌باشد.

Corrently any institute that does not benefit from information technology and communication networks, will gradually fall away from global communication. Therefore, various industries have shifted towards applying information technology in order to increas their quality and productivity in today’s competitive world. The health industry has also experienced such change. The health system has been constantly taking effective steps towards benefitting from the latest technologies to increase health and improve clinical anf financial results.
In an announcement made in the 55th public session of the United Nations in Sepptember, 2000, and accepted by 189 member countries, information and communication technology was introduced as an important tool for achieving all the goals of this organization such as reducing poverty, correcting educational and health services, easy access to governmental services and improving the society’s health level. Considering the importance of medical records as the most important information source in healthcare, we can claim that one of the best aspects of the application of information technology in the health system which has an key role in increasing the effectiveness of the health system, is the creation of electronic medical records.
The electronic health record tool is an electronic health record system that enables the healthcare personnel save time and money and improve the health condition of patients by organizing their records. Moreover, the diagnosis and treatment process will also be simplified. This tool combines traditional paper records with modern technological facilities for increasing the accuracy and speed of completing healthcare records. Using paper records has certain problems such as mistakes related to bad handwriting, prescriptions, and mistakes in writing words, icreased costs for copying forms, cards, and files, and most importantly the place needed for filing the records and the human resources needed for this task. Therefore, for overcoming these problems, electronic health records are suggested.
However, considering the sensitivity of the information, the confidentiality of the records is very important. Therefore, many countries have set policies int his regard. Maintaing the confidentiality of the records differs between societies considering their social, economical, scientific, political and cultural conditions.
