بررسي نيازهاي درماني و مراقبتي خانواده بيماران رواني پس از ترخيص از مركز روانپزشكي فرشچيان شهر همدان

Survey of Nursing and Care Needs of Patients with Mental Disorders than of Hospital Discharge

مقدمه و هدف : نيازهاي بيماران رواني و خانواده هاي آنان متعدد و متنوع و بر حسب مراحل مختلف بيماري متفاوت هستند. براي فراهم آوردن مراقبت همه جانبه براي برخي از بيماران رواني، طيف وسيعي از خدمات مورد نياز است. هدف از اين مطالعه تعيين نيازهاي درماني و مراقبتي بيماران رواني پس از ترخيص از بيمارستان فرشچيان شهر همدان در سال 1389 مي باشد. روش كار: اين پژوهش مطالعه اي توصيفي – مقطعي است. دراين مطالعه 200خانواده بيمار كه با تشخيص اسكيزوفرني ، اختلالات خلقي اوليه و اختلال وسواس-اجباري بستري و تحت درمان هاي روانپزشكي قرار داشتند يك ماه پس از ترخيص مورد ارزيابي قرار گرفتند. روش نمونه گبري بصورت دردسترس بود وابزار گردآوري داده ها پرسشنامه و شامل دو بخش بود: الف- سوالات مربوط به مشخصات دموگرافيك ب- سوالات در رابطه با ارزيابي نيازهاي درماني و مراقبتي بيماران پس از ترخيص نتايج: از نظر مشخصات دموگرافيك، ميانگين سني بيماران 5/38 سال و 68 درصد مرد و 32 درصد زن بودند. تشخيص هاي بيماري شامل 5/35 درصد اسكيزوفرني؛ 5/21 درصد افسردگي؛ 5/31 درصد اختلال دوقطبي تيپ I؛ 5/5 درصد اختلال وسواس- اجبار و 6 درصد تشخيص نامشخصي داشتند. ميانگين طول مدت بيماري 3/4 سال بود. اكثريت خانواده هاي بيماران رواني نيازهايي مانند مشاوره منظم توسط متخصص، ارايه مراقبت در جامعه، پيگيري درمان پس از ترخيص، آموزش به بيمار و خانواده، بازتواني رواني و اجتماعي را مهم مي دانستند. نتيجه نهايي: خدمات روانپزشكي و سلامت روان در جامعه بايد طيفي وسيعي از تسهيلات را براي رسيدگي به نيازهاي درماني و مراقبتي بيماران تامين كنند كه اين خدمات شامل مراقبت سرپايي، ايجادمراكز مبتني بر جامعه، مراقبتهاي حمايتي، مراكز نگهداري از بيماران، حمايتهاي شغلي، حرفه اي و بازتواني و تامين نيازهاي اساسي از قبيل پناهگاه و لباس مي باشد

Introduction & Objective: The needs of people with mental illness and their families are multiple and varied and differ at different stages of illness. A wide variety of services are required to provide comprehensive care for some of the people with mental illness. The aim of this study is to survey of nursing and care needs of patients with mental disorders than of hospital discharge. Materials & Methods: This was a descriptive cross-sectional study. The participants consist of 200 patientsʹ family, schizophrenic, Mood disorders and obsessive-compulsive, who were selected by stratified sampling. A 30 item questionnaire was used to gather data from the sample. It consisted of two parts: A- demographic factors B– questions related to nursing and cares needs. Results: The demographic findings included: 68% male and 32% female; and mean age of the participants was 38.5 years, 35.5% Schizophrenic, 21.5% depression, 31.5% bipolar disorders type I, 5.5% obsessive –compulsive and 6% . Mean of disease was 4.3 years. The needs such as counseling, community health care, fallow up, health teaching and social and psychological rehabilitation were important for the majority of the patients and their families. Conclusion: Community mental health services need to provide comprehensive and locally based treatment and care which is readily accessible to patients and their families. Services should be comprehensive in that they provide a range of facilities to meet the mental health needs of the population at large as well as of special groups. Ideally, services should include: nutrition; provision for acute admissions to general hospitals; outpatient care; community centers; outreach services; residential homes; respite for families and careers; occupational, vocational and rehabilitation supports; and basic necessities such as shelter and clothing.
