اثر الاستيك چين‌هاي آزاد كننده فلورايد بر تعداد استرپتوكوك موتانس بزاق و پلاك دنداني بيماران تحت درمان ارتودنسي

The Effect of Fluoride-releasing Elastomeric Chains on Streptococcus mutans Levels in Saliva and Dental Plaque in Orthodontic Patients

چكيده زمينه و هدف: مشخص شده حضور دستگاههاي ارتودنسي ثابت منجر به افزايش تعداد مطلق استرپتوكوك موتانس و همچنين افزايش درصد حضور اين باكتري در بزاق مي شود. اين امر سبب افزايش دمينراليزاسيون مينايي و پوسيدگي دنداني مي‌گردد. هدف از مطالعه حاضر ارزيابي اثر زنجيره‌هاي كشي (الاستيك چين‌هاي) آزاد كننده فلورايد بر تعداد استرپتوكوك موتانس بزاق و پلاك دنداني بيماران تحت درمان ارتودنسي مي باشد. روش بررسي: در اين كارآزمايي باليني تصادفي يك سو كور شصت بيمار تحت درمان ارتودنسي ثابت به صورت تصادفي به دو گروه مساوي تقسيم شدند. بيماران گروه 1 زنجيره هاي كشي معمولي و بيماران گروه 2 زنجيره هاي كشي آزادكننده فلورايد را دريافت كردند. چهار نمونه بزاق در ابتداي مطالعه و روزهاي هفتم، چهاردهم و بيست و هشتم و دو نمونه پلاك دنداني در ابتدا و انتهاي مطالعه از بيماران گرفته شد. زنجيره هاي كشي نيز در روز بيست و هشتم از دهان بيماران خارج شدند. همه نمونه ها جهت شمارش تعداد باكتري استرپتوكوك موتانس كشت داده شدند. اطلاعات جمع آوري شده توسط آزمونهاي آماري Repeated measures of ANOVA و Independent sample t test مورد مقايسه و بررسي قرار گرفتند. يافته‌ها: بررسيهاي آماري نشان‌ داد بين تعداد باكتري هاي موجود در سطح دو نوع زنجيره كشي اختلاف معناداري وجود ندارد(803/0= P). همچنين بين تعداد كلوني هاي استرپتوكوك موتانس طي چهار بار نمونه گيري از بزاق در دو گروه (301/0? P) و بين تعداد كلوني هاي استرپتوكوك موتانس موجود در پلاك دنداني دو گروه نيز اختلاف آماري معناداري مشاهده نشد (317/0= P). نتيجه‌گيري: استفاده از زنجيره هاي كشي آزاد كننده فلورايد تاثيري بر تعداد كلوني هاي استرپتوكوك موتانس موجود در پلاك دنداني و بزاق ندارد. كليد واژه‎ها: استرپتوكوك موتانس – زنجيره‌هاي كشي – فلورايد – پوسيدگي

Abstract Background and Aim: During fixed orthodontic treatments the absolute number and percentage of salivary Streptococcus mutans increase. This will cause increase of enamel demineralization and dental caries. The purpose of present study is to evaluate the effect of fluoride-releasing elastomeric chains in the streptococcus mutans levels in saliva and bacterial plaque. Materials and Methods: In this randomized clinical trial sixty patients, who were under fixed-orthodontic appliance treatments were selected and randomly divided into two groups of 30 each. Conventional elastomeric chains were used in group 1. As for the second group fluoride-releasing elastomeric chains were utilized. Four samples of saliva collected on days 0, 7, 14 and 28, and two plaque samples were collected on the beginning and finishing days in both groups. The fluoride-releasing and conventional elastomeric chains were removed on day 28. All samples were then used for microbial culture to count the Streptococcus mutans colonies. The results were analyzed statistically with the repeated measures Analysis of Variance (ANOVA) and Student t-tests. Results: The result of Streptococcus mutans (CFU) in saliva at each time point in the study showed no statistically significant difference between the two groups (p?0.301). Comparison of the effect of elastomeric chains on the Streptococcus mutans colonization in dental plaque and elastomeric chains surface, also did not show any statistically significant difference between the two groups (p= 0.317and 0.803 respectively). Conclusion: There was no clinical evidence that fluoride-releasing elastomeric chains were effective in reducing the formation and colonization of Streptococcus mutans in saliva and dental plaque.
