بررسي وضعيت انتشار استاف اوريوس مقاوم به آنتي بيوتيك در اتاق عمل بيمارستان گلپايگاني شهر قم

Evaluation of antibiotic resistant-Staphylococcous aureus in the operation room, Golpayegani Hospital, Qom

سابقه و هدف: استاف اوريوس بعنوان يك عامل اصلي عفونت اكتسابي در دنيا شناخته شده است. هم اكنون گسترش سويه هاي استاف اوريوس نيمه حساس و مقاوم به پني سيلين ها در برخي از بيمارستانها رو به افزايش است. هدف از اين مطالعه بررسي پراكندگي سويه هاي استاف اوريوس در قسمتهاي مختلف اتاق عمل در دو مرحله قبل و بعد از عمل و تعيين الگوي حساسيت آنتي بيوتيكي است. روش بررسي: آنتي بيوتيك هاي رايج بر عليه استاف اوريوس، جهت انجام آزمايشات مورد استفاده قرار گرفت. تعيين حداقل غلظت بازدارنده رشد مطابق استانداردهاي كميته ملي آزمايشگاههاي كلنيكي (NCCLS) و با روش رقت در آگار انجام شد. اين مطالعه از نوع توصيفي بود كه در نيمسال اول 1384 در اتاق عمل بيمارستان گلپايگاني قم انجام گرفت. آناليز آماري به كمك آزمونهاي T و paired t-test انجام شد. يافته ها: فراواني انتشار استاف اوريوس در شش ماه اول سال قبل از عمل نسبت به بعد از عمل كمتر و بيشترين فراواني در طول ماههاي تابستان بدست آمد. قبل از عمل بيشترين تعداد استاف اوريوس بترتيب از نمونه هاي تي، جاروب و درز كف اتاق ولي بعد از عمل بيشترين تعداد از تي، درز اتاق و ماسك بيهوشي جدا گرديد. تمام سويه هاي جدا شده از مرحله قبل و بعد از عمل نسبت به آنتي بيوتيك هاي ونكومايسين و متي سيلين حساسيت نشان داده در حاليكه نسبت به آنتي بيوتيك هاي كربوكسي سيلين، تتراسيكلين، سفازولين و پني سيلين بيشترين مقاومت در سويه هاي جدا شده در مرحله بعد از عمل مشاهده گرديد. نتيجه گيري: نظر به فراواني بيشتر سويه هاي مقاوم به آنتي بيوتيك در ماههاي گرم سال، احتمالا دما و ناتواني ضدعفوني كننده ها در حذف بيوفيلم باكتري در مكانهايي مثل تي و كف اتاق، نقش مهمي در انتخاب سويه هاي مقاوم دارند. بنابراين براي حذف كامل سويه هاي مقاوم به آنتي بيوتيك، نمونه گيري از تمام قسمتهاي اتاق عمل و بكارگيري ضدعفوني كننده موثر بلافاصله بعد از عمل بويژه در فصل گرم سال پيشنهاد مي گردد.

Background: Staph aureus continues to be a major cause of community-acquired and health-care related infection worldwide. The emergence of high levels penicillin resistance Staph followed by the development and spread of strains resistant to the semi synthetic penicillin’s has made the therapy of staphylococcal disease a global challenge. At present, vancomycin- intermediate and resistant Staph aureus has been spreading in some hospitals. The purpose of the present study was to determine the distribution of S. aureus at various sites of eye surgery room in pre and post operation and its antibiotic sensitivity at Golpayegani hospital. Materials and methods: Common antibiotic against S. aureus used for antibiotic testing. The determination of MIC was performed with the agar dilution method according to the procedure outlined by NCCLS. This study was descriptive and was performed in Golpayegani hospital of Qom during the first semester of 2005. The statistical analysis was done by using T and paired T- Test. Results: The frequency of Staph aureus was lower in preoperative setting during the Spring and Summer seasons. In preoperative sampling, most of the Staph aureus were isolated from broom and floor, while in postoperative sampling, most of the isolates were from broom, floor and anesthetic mask. All of the isolates revealed a totally different antimicrobial susceptibility pattern so that all of the isolates were susceptible to Vancomycin and Methicilin, however, postoperative isolates showed resistance to Carboxycilin, Tetracyclin, Cephazolin and Penicillin. Conclusion: According to the higher frequency of antimicrobial resistant isolates during warm months, probably insufficient disinfectant technique and temperature may play a major role in bacterial resistance. Thus, for effective removal of all bacteria, immediate application of antiseptic agents after surgery is recommended.
