Trade in Health Services via Four Modes in Asian Countries: Challenges and Opportunities

امروزه تجارت خدمات سلامت به سرعت در حال رشد است و فن آوري اطلاعات و ارتباطات اين روند را آسان تر مي سازد. خدمات سلامت طبق از طريق چهار حالت معامله م يشوند. ،(General agreement on trade in services يا GATS) موافقت نامه ي عمومي تجارت خدمات مهاجرت متخصصين سلامت يكي از اشكال تجارت خدمات سلامت است كه به دليل تغييرات دموگرافيك، سالمندي جمعيت و افزايش هزينه هاي خدمات سلامت در كشورهاي با درآمد بالا به عنوان عوامل افزايش دهند هي تقاضا و توسعه ي صادرات، افزايش درآمد ارزي و ارتقاي دانش و مهارت هاي كاركنان به عنوان عوامل ترغيب كننده ي عرضه، روندي رو به رشد داشته است. با اين كه مهاجرت متخصصين سلامت مزايايي را براي كشورها در بردارد، اما مي تواند چالش هايي نيز به همراه داشته باشد. هدف اين مقاله، شناسايي فرص تها و چال شهاي حاصل از مهاجرت متخصصين سلامت دركشورهاي آسيايي بود. در اين مقاله، تجارت خدمات سلامت به عنوان يكي از پيچيده ترين چالش هاي نظام سلامت، انواع آن و وضعيت كنوني كشورهاي آسيايي در رابطه با حالت چهارم معاملات خدمات سلامت، مورد بررسي قرار گرفته و فرص تها و چال شهاي حاصل از مهاجرت متخصصين سلامت بيان گرديده است. بررسي ها نشان داد كه كشورهاي وارد كننده يا صادر كننده ي خدمات سلامت، با فرصت ها و تهديدهاي يكساني روبه رو نيستند. مهاجرت به كشورهاي با درآمد بالاتر، فرصتي براي كار و شرايط بهتر جهت متخصصين سلامت فراهم م يكند و يك منبع درآمد براي كشور صادركننده است. اما فقدان سياست هاي صريح و روشن در اين زمينه، منجر به تخليه ي كشور مبدا از نيروي كار متخصص -كه به از آن ياد مي شود-، كاهش نيروي كار ملي و از دست دادن سرمايه هاي صرف شده در آموزش اين كاركنان مي گردد. « فرار مغزها » عنوان واژه هاي كليدي: آسيا؛ تجارت خدمات سلامت؛ چالش ها؛ فرصت ها

Information and communication technology have made it easier to trade health services and the business is rapidly growing. Health services under the General Agreement on Trade in Services (GATS) are traded through four modes. Migration of health professionals, the 4th mode of trade in health services, has had a growing trend due to demographic changes, aging populations and rising health care costs in high-income countries as factors that increase demand, and expanding exports, increase foreign exchange incomes, increase knowledge and skills of employees supply as factors encouraging supply. Although migration of health professionals to countries has advantages, it can be a challenge too. This study reviewed the opportunities and challenges of migration of health professionals from the Asian countries. Trade in health services as one of most complex challenges of health system, its various types, and the current situation of Asian countries dealing with the fourth type were studied and opportunities and challenges of migration of health professionals were expressed. Studies showed that countries importing or exporting health services are not faced with identical opportunities and threats. Migration to higher income countries has the opportunity to work and better conditions for health professionals and provides a source of income for the exporting country. However, lack of explicit policies in this area will lead to the depletion of the country of origin from skilled labor which reduces labor and leads to loss of national wealth spent on educating the staff.
