همراهي سيستينوزيس با سندرم دوين (گزارش مورد)

Association of Cystinosis with Duane Syndrome: A Cease Report

هدف: گـزارش يك بيمـار مبتـلا به سيستينوزيس، پرفشاري ايديوپاتيك داخل مغزي و سندرم دوين تيپ يك همراه يكديگر. معرفي بيمار: بيمار دختر 15 ساله‌اي بود كه مورد شناخته شده سيستينوزيس با درگيري كليوي محسوب مي‌شد و 6 سال قبل مورد پيوند كليه قرار گرفته بود كه با شكايت از تغييرات شكاف پلكي در نگاه‌هاي جانبي از بدو تولد و سردرد از 6 ماه قبل مراجعه كرد. وي فردي چاق و كوتاه قد بود. در معاينه چشمي رسوبات كريستال‌هاي سيستين در قرنيه و ملتحمه مشاهده مي‌شد. حركت هر دو چشم به خارج محدود بوده و در نگاه به داخل شكاف پلكي تنگ شده و رتراكسيون كره چشم ديده مي‌شد. در معاينه فوندوس هر دو چشم، عصب بينايي حدود محو داشته و متورم به نظر مي‌رسيد. در بررسي مايع مغزي نخاعي فشار داخل مغز افزايش يافته بود. يك ماه پس از شروع استازولاميد ادم عصب بينايي و سردرد بهبودي چشم‌گيري نشان داد. بيمار سه ماه پس از مراجعه، به خاطر مشكلات متابوليك در بخش مغز و اعصاب فوت شد. نتيجه‌گيري: در اين مقاله همراهي سه بيماري نادر سيستينوزيس و سندرم دوين دو طرفه تيپ I و نيز افزايش فشار داخل مغز ايديوپاتيك گزارش شد. توصيه ما آن است كه اين همراهي هر چه زودتر شناخته شود تا بتوان از عوارض و مرگ ناشي از آن‌ها جلوگيري كرد.

Purpose: To report a case of cystinosis and idiopathic intracranial hypertention associated with Duane syndrome type I. Case Report: A 15 years old girl and a known case of nephrotic cystinosis presented to eye clinic with complaints of lid fissure changes on horizontal gaze and headache since 6 months ago. She was obese and had short stature. She had history of kidney transplantation 6 years ago. On physical examination both eyes were orthophoric in primary position but limitation of movements in abduction, globe retraction and narrowing of the palpebral fissure in adduction was present slit lamp examination revealed deposition of cystine crystals in the conjunctiva and corneal stroma of both eyes. On fundus examination both optic discs had blurred margin and were edematous. In lumbar puncture (LP) intracranial pressure was high. One month after starting acetazolamide, the papilledema was decreased significantly and her headache improved. After 3 months the patient passed away because of metabolic problems in neurologic ward. Conclusion: Herein we report a rare case of concurrent cystinosis, bilateral Duane syndrome type I and idiopathic intra cranial hypertention.
