تاثير تغيير موقعيت منديبل برروي قابليت اطمينان و تجديدپذيري اندازه گيريهاي خطي در تصاوير CBCT

Effect of Mandible Orientation on Reliability and Reproducibility of Linear Measurements Using Cone-Beam Computed Tomography

خلاصه: سابقه و هدف: دقت ابعادي تصاوير CBCT پيش نياز مطمين در درمان هاي ايمپلنت هاي دنداني است. هدف پژوهش حاضر تعيين تاثير تغيير موقعيت منديبل بر روي دقت اندازه گيريهاي خطي روي تصاوير CBCT مي باشد. مواد و روش‌ها: در اين مطالعه تجربي 12 فاصله خطي روي يك منديبل خشك به وسيله كوليس ديجيتال سه مرتبه توسط دو مشاهده گر اندازه گيري شد. سپس منديبل توسط دستگاه ( CBCTscanora 3D) در پنج حالت اسكن شد: موازي افق، تمايل به پايين، به بالا، تمايل لترالي و حالت چرخيده. فواصل خطي روي هر يك از تصاوير در حالات مختلف توسط دو مشاهده گر در دو زمان(در هر نوبت سه بار) اندازه گيري شد. خطاي زير 10 درصد طبق مرجع به عنوان حاشيه امن تلقي شد. سپس توسط نرم افزار تمايلهاي اعمالي اصلاح و اندازه گيريها تكرار گشت. آزمون Tزوجي و همبستگي پيرسون براي آناليز آماري استفاده شد. يافته‌ها: 4/88 درصد اندازه گيريهاي روي تصاوير كوچكتر از اندازه گيريهاي انجام شده روي منديبل بود. ميزان اختلاف براي تمايلهاي مختلف بين 01/0±65/2 ميلي متر بدست آمد. اندازه گيريها (به جز شش مورد) خطاي زير 10 درصد داشتند كه در حاشيه امن بود. پس از اصلاح تمايلها، خطاي بيش از يك ميليمتر تنها در يك مورد مشاهده شد و درصد خطا از 8 درصد تجاوز نمي كرد. از نظر تكرارپذيري، همبستگي بالا بين مشاهده گرها و نيز بين دو بار خوانش يك مشاهده گر بدست آمد. Abstract: Background and Aim: The aim of the present study was to assess the effect of mandibular inclination on accuracy of linear measurements performed on CBCT scans. Materials and Methods: In this descriptive analytical study, 12 linear dimensions between anatomical landmarks were measured on a dried human mandible. The measurements were obtained with a digital caliper three times by two observers. The mandible was scanned using a cone-beam CT(scanora 3D) with the base of mandible positioned in 5 different inclinations: horizontal, tilted downwards, tilted upwards, laterally tilted and rotated. Computer measurements of the dimensions were obtained twice by two observers, three times at each session. Then, the inclinations were corrected by the software and all the measurements were repeated. Paired T-test and Pearson correlation test were used to analyze the data. Results: In 88.4% of the samples, the mean computer measurements were smaller than the mean manual caliper measurements. The mean difference was 0.01-2.65 mm for different inclinations. The percentage error was below 10% (except for 6 cases) which is considered to be the safe margin in studies. Following correction of the inclinations, the mean absolute error exceeding 1 mm was observed in only one case and the percentage error was less than 8%. Regarding the reproducibility high inter and intra examiner correlation was observed(r > 0.9). High inter and intra examiner correlation was observed with respect to the reproducibility of the results(r > 0.9). Conclusion: Head orientation during CBCT scanning is not as crucial as in CT scan especially when the inclinations are corrected via software before the measurements.
