Fabrication of a Hybrid Screw-Retained and Cement-Retained Implant Prosthesis: A Case Report

سابقه و هدف: تكنيك‌هاي پروتزي گوناگوني براي درمان بيماران ايمپلنتي بكار مي‌روند. يكي از اين تكنيك‌ها ساخت پروتز سمان‌شونده روي يك فريم ورك پيچ‌شونده مي‌باشد. اين نوع پروتز بر روي ايمپلنت‌هايي كه در موقعيت نامناسب يا بطور زاويه‌دار قرار گرفته‌اند، بكار برده مي‌شود. مزيت اين روش در سهولت بازيابي، تعمير و مراقبت از پروتز است. هدف از گزارش مورد حاضر توضيح روش ساخت پروتزهاي ايمپلنت ساپورت مي‌باشد كه در آن روكش‌هاي مجزا، روي فريم ورك پيچ‌شونده‌اي سمان مي‌شوند. اين اسكلت فلزي به منظور ساپورت بافت نرم با پرسلن رنگ لثه پوشيده شده است. گزارش مورد: بيمار خانمي بود 56 ساله با بي‌دنداني كامل كه بواسطه استفاده طولاني مدت از دنچر به تحليل شديد استخواني مبتلا بود بطوري كه حتي پس از بازسازي استخواني وسيع با استخوان ايلياك نيز فرم نرمال ريج ايجاد نشده بود، بنابراين از روش ساخت توام پروتز پيچ شونده وسمان شونده براي درمان ايمپلنت بيمار استفاده گرديد. نتيجه‌گيري: اين تكنيك مزاياي دو روش پيچ‌شونده وسمان‌شونده را از نظر عملكرد و زيبايي داراست. همچنين علي‌رغم مكان نامناسب كانال دسترسي پيچ در فريم، بررسي ايمپلنت‌ها با بازكردن اسكلت فلزي همواره قابل انجام بوده است. در عين حال روكش‌هاي ساخته شده نيز بدون باز كردن فريم ورك قابل تعويض يا تعمير مي‌باشند.

Objective: Various prosthodontic techniques are used for treatment of implant patients. Fabrication of a cement-retained prosthesis on a screw-retained framework is among these techniques. This type of prosthesis is usually indicated for implants that have been insertedobliquely or in an improper position. Easy retrieval, repair and maintenance are the main advantages of this type of prosthesis. The present articledescribes a method for fabrication of implant supported prosthesis. In this prosthesis, separate crowns are cemented on a screw-retained framework. The metal framework is coated with gingival-colored porcelain in order to support soft tissue. Case: Our patient was a completely edentulous 56 year-old female suffering from severe bone loss as the result of long term use of dentures. The bone resorption was so severe that the normal ridge contour could not be restored even after extensive ridge augmentationusing iliac crest bone graft. Thus, a combination of cement-retained and screw-retained prostheses was selected as the treatment of choice for this patient. Conclusion: This technique combines the advantages of both screw-retained and cement-retained prostheses in terms of function and esthetics. Also, despite the hardly-accessible location of the screw access channel in the framework, assessment of implants is always feasible by unscrewing the metal framework. Additionally, the fabricated crowns can be easily repaired or replaced without the need for unscrewing the framework.
