Strategic Training Methods Regarding the Physicians and Nursesʹʹ Clinical Relationships with the Patients Suffering from Cancer

عليرغم پيشرفتهاي قابل توجه علم پزشكي، سرطان(Cancer) يكي از علل مهم مرگ و مير در دنيا مي باشد و به عنوان يكي از مهمترين بيماريهاي قرن حاضر و دومين علت مرگ و مير بعد از بيماريهاي قلب و عروقي مطرح است . ميزان مرگ ناشي از سرطان در كل دنيا در سال 2007 حدود 9/7 ميليون نفر (حدود 13% كل موارد مرگ و مير) تخمين زده مي‌شود. فرد متعاقب تشخيص آن دچار اضطراب و افسردگي ناشي از ترس غيرواقعي از مرگ و كاهش انرژي اجتماعي مي‌گردد، بطوري كه ضرورت بستري مكرر و نگراني هاي مداوم براي بيماران و خانواده هاي آنها ، فرد را به سمت و سوي اختلالات رواني مي كشاند. بررسي مسايل اخلاقي در بيماران مبتلا به سرطان بسيار چالش برانگيز است و ممكن است اين بيماران در وضعيت روحي مناسبي جهت تصميمگيري در رابطه با افزايش طول زندگي و يا كاهش كيفيت زندگي و عوارض جانبي آن نباشند. با اين حال در چنين مواردي خواسته بيمار بايد مورد احترام قرار گيرد. هدف از اين مقاله بررسي ارتباط باليني مناسب پزشكان و پرستاران با افراد مبتلا به سرطان مي باشد. اين ارتباط بين پزشك و بيمار در تمامي مراحل بيماري از انجام آزمايشات غربالگري سرطان گرفته تا برقراري اعتماد كامل بين بيمار و پزشك را شامل مي‌شود. همچنين مساله اتانازي در فرهنگ‌هاي مختلف مورد بررسي قرار گرفته و سطوح مختلف تعامل بين پزشك و بيمار بيان شده و در نهايت راهكارهايي چند در جهت تعامل هر چه بهتر و موثرتر پزشكان و پرستاران با اين گونه بيماران پيشنهاد مي‌گردد.

In spite of considerable advances in medical sciences, cancer is one of the major causes of death in the world. Cancer is one of the most important diseases of the time and is considered as the second cause of mortality after cardiovascular diseases. The rate of cancer-related deaths in the world was estimated to be about 7.9 million individuals (about 13% of all deaths) in 2007. Cancer patients suffer from depression, anxiety, unrealistic fear of death, and reduction of social energy. Moreover, continuous hospitalization and concerns for the patients and their families lead these patients towards mental disorders. Ethical issues are highly challenging in cancer patients. Some cancer patients may not be in an appropriate mental state to decide regarding increasing the life span or decreasing the life quality and its complications. In any event, the patients’ decision must be respected. The present study aimed to investigate the clinical relationship between the cancer patients and the physicians as well as nurses. Such a relationship involves all the stages from screening to creation of trust between the patients and the physicians. Furthermore, euthanasia has been investigated in various cultures and different levels of interaction between the patients and physicians have been assessed. Finally, strategies have been recommended for improving the physicians’ and nurses’ interaction with these patients. Keywords: Cancer, Relationship, Quality of life
