بررسي مقايسه‌اي فاصله‌ي اپكس دندان سانترال ماگزيلا تا كف حفره‌ي بيني در راديوگرافي‌هاي لترال سفالومتري، پانوراميك و پري اپيكال

Comparative evaluation of the distance between the apex of the maxillary central incisor and the nasal cavity floor on lateral cephalometric, panoramic and periapical radiographs

چكيده مقدمه: كاربرد ايمپلنت‌هاي دنداني در درمان بيماران بدون دندان روبه افزايش است، كه نيازمند بررسي‌هاي راديوگرافي وسيع مي‌باشد و اغلب جهت ارزيابي ارتفاع، پهنا و ساختار استخوان در محل مورد نظر براي جاي‌گذاري ايمپلنت، نياز به استفاده از چندين تكنيك تصويربرداري وجود دارد. مطالعه‌ي حاضر با هدف مقايسه‌ي ميانگين فاصله‌ي كف حفره‌ي بيني تا اپكس دندان سانترال ماگزيلا در راديوگرافي‌هاي لترال سفالومتري، پانوراميك و پري اپيكال طراحي گرديد. مواد و روش‌ها: در اين مطالعه‌ي توصيفي– تحليلي و بدون جهت، راديوگرافي‌هاي 16 نفر از افراد مراجعه كننده به كلينيك دندان‌پزشكي افضل شهر اصفهان كه داراي هر سه راديوگرافي پري اپيكال از انسيزورهاي سانترال ماگزيلا، پانوراميك و لترال سفالومتري بودند، جمع‌آوري شدند. با استفاده از كوليس فاصله‌ي اپكس دندان سانترال چپ ماگزيلاي اين افراد تا كف حفره‌ي بيني بررسي گرديد. تمامي داده‌ها توسط آناليز‌هاي آماري paired t-test و رگرسيون در نرم‌افزار آماري SPSS مورد تجزيه وتحليل آماري قرار گرفت (05/0 = ?). يافته‌ها: مقايسه‌ي ميانگين فاصله‌ي اپكس دندان سانترال ماگزيلا تا كف حفره‌ي بيني در راديوگرافي پري اپيكال و مقدار واقعي (استاندارد طلايي) به‌دست آمده با كمك راديوگرافي لترال سفالومتري، تفاوت معنادار داشت (001/0 ? p value) و از مقدار واقعي كم‌تر بود. مقايسه ميانگين فاصله‌ي اپكس دندان سانترال ماگزيلا تا كف حفره‌ي بيني در راديوگرافي پانوراميك و مقدار واقعي به‌دست آمده با كمك راديوگرافي لترال سفالومتري تفاوت معنادار داشت (017/0 = p value) و از مقدار واقعي كم‌تر بود. نتيجه‌‌گيري: با در نظر گرفتن محدوديت‌هاي اين مطالعه، مقادير به‌دست آمده در هر دو راديوگرافي پانوراميك و پري اپيكال در اندازه‌گيري ميزان فاصله‌ي اپكس دندان سانترال ماگزيلا تا كف حفره‌ي بيني كم‌تر از مقدار واقعي است. كليد واژه‌ها: راديوگرافي پانوراميك، فك بالا، اندازه‌گيري

Abstract Introduction: Use of dental implants is increasing in the treatment of edentulous patients, necessitating extensive radiographic assessments to evaluate height, width and structure of the bone for placement of implants, which needs several radiographic techniques. This study was designed to evaluate the mean distance between the nasal cavity floor and the apex of maxillary central incisor on lateral cephalometric, panoramic and periapical radiographs. Materials and methods: In this random descriptive/analytical study 16 patients referring Afzal Dental Clinic in Isfahan city, who all had taken lateral cephalometric, panoramic and periapical radiographs of maxillary central incisors were included. The distance between the apex of maxillary central incisor and the nasal cavity floor was measured by a caliper. Data was analyzed by SPSS via paired t-test and regression statistical test (?=0.05). Results: According to data analysis, mean distance between the apex of maxillary central incisor and the nasal floor on periapical radiographs had a significant difference with the actual distance (gold standard) measured on lateral cephalometric radiographs (p value < 0.001) and it was less than the actual size. The same measurement achieved on panoramic radiographs and actual distance measured on lateral cephalometry showed a significant difference (p value = 0.017) and it was less than the actual size. Conclusion: Under the limitations of this study, it was concluded that both measurements made on panoramic and periapical radiographs underestimated the nasal cavity floor distance to the apex of maxillary central incisors.
