بررسي روايي روش Willems بر اساس توالي تكامل دندان‏هاي مولر و پرمولر در افراد

Validity of Willems Method Based on Molar and Premolar Development in 3-18 year old Individuals in the City of Isfahan

مقدمه: دقت روش‏هاي Willems و Demirjian مي‏تواند تحت تاثير الگوهاي متفاوت كلسيفيكاسيون دندان‏هاي مولر و پرمولر قرار گيرد. هدف از اين مطالعه تعيين دقت روش Willems در جامعه مورد مطالعه و بررسي تاثير الگوهاي متفاوت كلسيفيكاسيون دندان‏هاي مولر و پرمولر بر اساس اين روش بود. مواد و روش‏ها: در اين مطالعه توصيفي-مقطعي، 152 راديوگرافي پانوراميك 47 پسر و 105 دختر 3 تا 18 ساله استفاده شد. سن دنداني نمونه‏ها به روش Willems و Demirjian تعيين شد. بر اساس تكامل دندان‏هاي پرمولر اول و دوم و مولر دوم گروه بندي زير انجام شد: گروه اول: مرحله تكاملي پرمولر اول و دوم همزمان، گروه دوم:‌ مرحله تكاملي پرمولر اول جلوتر از پرمولر دوم، گروه سوم: مرحله تكاملي پرمولر دوم با مولر دوم همزمان است و گروه چهارم: مرحله تكاملي پرمولر دوم جلوتر از مولر دوم. داده‏ها وارد نرم افزار SPSS با ويرايش 17 شد و توسط آزمون t زوج بررسي شد (05/0?=) يافته‏ها: روش Demirjian باعث تخمين بيش از حد سن (5/0 سال) و روش Willems باعث تخمين كمتر سن (2/0 سال) شد، كه نشان‏دهنده دقت بيشتر روش Willems بود. روش Willems در كل جامعه در گروه يك، در دختران در گروه يك و در پسران در گروه دوم، بيشترين دقت را نشان داد. روش Demirjian در كل جامعه و دختران در گروه دوم و در پسران در گروه سوم بيشترين دقت را نشان داد. نتيجه گيري: اين تحقيق نشان داد كه دقت روش Willems بيش‏تر از روش Demirjian است و طرح‏هاي متفاوت از تكامل دنداني بر دقت اين روش‏ها تاثير مي‏گذارد.

Introduction: The accuracy of Willems and Demirjian’s methods can be affected by different patterns of calcifications of molars and premolars. Materials & Methods: In this descriptive-cross sectional study, 152 panoramic radiographs of 47 boys and 105 girls aged 3 to 18 were used. The dental age of samples was determined by Willems and Demirjian’s methods. Based on the developmental of first and second premolars and second molar the following ranking was made: 1. The development stage of the first premolar and the second premolar is the same. 2. The development stage of the first premolar is earlier than the second premolar. 3. The development stage of the second premolar and the second molar is the same. 4. The development stage of the second premolar is earlier than the second molar. Data were entered into SPSS 17 software and analysed by paired t-test. Results: Demirjian?s method overestimated the age of the teeth (0.5 years) and Willms? method under estimated the age of the teeth (0.2 years) that shows the higher accuracy of the Wilelms? method. The Willems method in the first group of the whole population,in the girls of the first group and in the boys of the second group showed the highest accuracy. Demirjian’s method in the whole population and in the girls of the second group and in the boys of the third group showed the highest accuracy. Discussion: This research showed that the accuracy of Willem?s method is higher than Demirjian?s, and different patterns of tooth development affect the accuracy of these methods and will help the forensic dentist to be more ensured.
