آزمون تصور ذهني از راستاي عمود: تاثير زاويه ي اوليه و زمينه ي بينايي

Subjective visual vertical test: the effect of preset angle and visual background

مقدمه و اهداف آزمون "تصور فرد از راستاي عمود (SVV)" درك ذهني فرد از راستاي عمود را تخمين مي زند. مطالعات پيشين نشان دادند كه افراد داراي عملكرد دهليزي هنجار، خطّ شاخص SVV را در محدوده ي 2 درجه از محور عمودي واقعي (صفر درجه) تنظيم مي كنند. هدف از اين مطالعه، بررسي تاثير زاويه ي اوليه بر مقدار SVV در زمينه ي بينايي ثابت و متحرك و همچنين مقايسه ي تاثير چرخش ساعتگرد و پادساعتگرد زمينه ي بينايي بر مقدار SVV بود. مواد و روش ها از 50 فرد بزرگسال هنجار (19 مرد و 31 زن) در محدوده ي سني 18 تا 40 سال درخواست شد كه يك خط نوراني را در 6 وضعيت مختلف بصورت عمود تنظيم كنند: 2 بار در زمينه ي بينايي ثابت و 4 بار در زمينه ي بينايي متحرك. در اين مطالعه از دستگاه SVV كمپاني Synapsys استفاده شد. يافته ها از يك سو، جهت زاويه ي اوليه تاثيري همانطرفي بر جهت انحراف SVV در زمينه ي بينايي ثابت و متحرك داشت به اين معني كه اگر زاويه ي اوليه به سمت راست انحراف داشت، انحراف SVV نيز به سمت راست و اگر زاويه ي اوليه به سمت چپ انحراف داشت، انحراف SVV نيز به سمت چپ بود (P.value < 0.001). از سوي ديگر، در SVV با زمينه ي بينايي متحرك نيز جهت چرخش زمينه ي بينايي تاثيري همسو و چشمگير بر انحراف SVV داشت يعني چرخش ساعتگرد، SVV را به سمت راست و چرخش پادساعتگرد، آنرا به سمت چپ منحرف مي كرد (P.value < 0.001). نتيجه گيري براي تفسير درست و تشخيص دقيق اختلالات وستيبولار با استفاده از آزمون SVV در مجموعه آزمون ارزيابي سيستم وستيبولار، بايد به تاثير زاويه ي اوليه و چرخش زمينه ي بينايي بر نتايج آن توجه داشته باشيم. واژگان كليدي تصور ذهني از راستاي عمود، زمينه ي بينايي ثابت، زمينه ي بينايي متحرك، زاويه ي اوليه، جهت چرخش زمينه ي بينايي

Background and Aim: Abstract Subjective Visual Vertical (SVV) test estimates mental perception of vertical orientation. Previous researches showed that individuals with normal vestibular function align the SVV line indicator within 2 degrees of true vertical (0 degrees). The aims of this study were to evaluate the effect of preset angle on SVV value in static and dynamic visual background and also to compare the effect of clockwise and counterclockwise rotation of visual background on SVV value. Material & Method: Fifty normal adults (19 men and 31 women), with ages ranged from 18 to 40 years were asked to set a luminous bar as vertical in six different conditions: two times in static visual background condition and four times in dynamic visual background condition. Synapsys SVV instrument was used in this study. Results: On one hand, the direction of preset angle had ipsilateral effect on deviation of SVV with static and dynamic background. It means that right preset angle made rightward deviation of SVV and left preset angle made leftward deviation of SVV (P < 0.001). On the other hand, the direction of visual background rotation had significant and ipsilateral effect on SVV with dynamic background. It means clockwise rotation made rightward deviation of SVV and counterclockwise rotation made leftward deviation of SVV (P < 0.001). Conclusion: For correct interpretation and precise diagnosis of vestibular disorders using the SVV test within the vestibular test battery, we should consider the effect of preset angle and rotation of visual background on the results.
