Clinical Accuracy of Ipex Apex Locator for Measurement of Root Canal Length of Primary Molars

تعيين دقت آپكس ياب الكترونيكي در اندازه‌گيري طول كانال دندانهاي مولر شيري

چكيده زمينه و هدف: راديوگرافي معمولترين روش تعيين طول ‌در درمانهاي اندودنتيك كودكان مي‌باشد كه استفاده از آن مشكلاتي در بر دارد. آپكس ياب الكترونيكي، وسيله‌اي براي اندازه‌گيري طول كانال مي‌باشد. هدف از اين مطالعه، ارزيابي دقت وسيله‌ الكترونيكي تعيين طول كانال دندان (Ipex) در اندازه‌گيري طول كانال دندانهاي شيري در شرايط باليني مي‌باشد. روش بررسي: در اين مطالعه باليني، از پنجاه كانال ريشه دندانهاي مولر شيري كه محكوم به خارج كردن بودند، استفاده گرديد.پس از انجام بي‌حسي و حذف پوسيدگي، حفره دسترسي ايجاد شده و طول كانال توسط دستگاه آپكس ياب Ipex اندازه گيري شد، بعد از ثبت طول كانال، دندانها خارج شده و طول حقيقي كانال به وسيله فايل شماره 15 تعيين گرديد. اطلاعات با استفاده از آزمون T زوج و ضريب همبستگي درون گروهي و آناليز رگرسيون خطي مورد تجزيه و تحليل قرار گرفتند. يافته‌ها: دقت دستگاه Ipex براي تعيين دقيق طول حقيقي كانال 20% و در فاصله 5/0± ميلي‌متري از انتهاي ريشه 66% و در فاصله 1± ميلي‌متر از انتهاي ريشه 80% به دست آمد، آزمون T زوجي اختلاف مقدار واقعي طول كانال و طول اندازه‌گيري شده با Ipex را معني‌دار نشان داد (001/0P.v < )، آزمون همبستگي Pearson نشان داد كه يك رابطه خطي مستقيم‌ قوي بين طول اندازه‌گيري شده Ipex و طول حقيقي كانال وجود دارد. (957/0=r ) نتيجه‌گيري: دستگاه Ipex به طور غالب تمايل به كوتاهتر نشان دادن طول كانال نسبت به طول واقعي آن دارد. از آنجا كه طول كاركرد كوتاهتر از طول واقعي است، اين دستگاه براي درمان اندودنتيك دندانهاي شيري مي‌تواند مدّ نظر قرار بگيرد. كليد واژه‎ها: آپكس ياب ، مولر شيري ، طول كانال

Abstract Background and Aim: Radiography is the most commonly used technique for root canal length determination; but its application in pedodontics is difficult due to problems name-ly the radiation hazards, superimposition of permanent teeth buds and uncooperative children. Electronic apex locator (EAL) is used for determination of root canal length. The aim of this study was to assess the accuracy of an Electronic Apex Locator (Ipex) in measuring the root canal length of primary teeth in-vivo. Materials and Methods: This clinical study was conducted on 50 root canals of primary teeth that had to be extracted. After local anesthesia induction and caries removal, access cavities were prepared and the length of root canals was measured using Ipex Electronic Apex Locator. After the extraction of teeth, the actual length of canals was measured using a #15 K file from a reference point. Data were analyzed using paired t-test and intra-class correlation coefficient. Results: The accuracy of Ipex was 20% for accurate determination of actual root canal length, 66% for estimation within ±0.5mm of the apex and 80% for estimation within ±1 mm of the apex. Paired t-test revealed a significant difference between the actual root canal length and the length displayed by Ipex ( P < 0.001). A direct strong correlation ex-isted between the length measured by Ipex and the actual root canal length (r=0.957). Conclusion: Ipex had a tendency to underestimate the actual root canal length. Since the working length is shorter than the actual root canal length, Ipex may be used for endodontic treatment of primary teeth.
