فرايند تطابق در دريافت كنندگان پيوند كليه: يك مطالعه گراندد تيوري

Coping process in renal transplant recipients: A grounded theory study

چكيده زمينه و هدف: پيوند كليه به عنوان درمان انتخابي و موثرترين روش جايگزين در بيماري كليوي در مرحله انتهايي به حساب مي‌آيد. اگرچه پيوند مي‌تواند موجب بهبود كيفيت زندگي بيماران گردد، اما آنان را با چالش‌ها و نگراني‌هاي جديدي مواجه خواهد كرد. از آن‌جايي كه اطلاعات كمي در مورد فرايند تطابق در اين بيماران وجود دارد، فهم چگونگي تطابق با كليه پيوندي، تدارك دهندگان مراقبت را قادر خواهد كرد تا بتوانند برنامه‌هاي مراقبت مناسب و جامعي را برنامه‌ريزي و اجرا نمايند. هدف اين مطالعه، تبيين فرايند تطابق در دريافت كنندگان پيوند كليه بود. مواد و روش‌ها: در انجام اين تحقيق، روش كيفي بر اساس نظريه داده‌بنياد براي تبيين فرايند تطابق مورد استفاده قرار گرفت. در ابتدا، مشاركت كنندگان به صورت مبتني بر هدف و از بين دريافت كنندگان پيوند انتخاب شدند و به تدريج كار با نمونه‌گيري نظري ادامه پيدا كرد. 11 بيمار دريافت كننده پيوند از بخش پيوند بيمارستان افضلي‌پور كرمان و 3 نفر از اعضاي خانواده آنان (14 نفر) طي سال 91-1390 مورد مصاحبه قرار گرفتند. براي تجزيه و تحليل داده‌ها، نسخه پيشنهادي Strauss و Corbin مورد استفاده قرار گرفت. يافته‌ها: سه طبقه درك واقعيت پيوند، راهبردهاي مقابله‌اي و يادگيري زندگي چالشي از تحليل داده‌ها به دست آمد. يافته‌ها دلالت بر اين داشت كه اين فرايند تدريجي و آرام بوده و در هر مرحله داراي ويژگي‌هاي خاص مي‌باشد. طبقه مركزي استخراج شده «تلاش مداوم و هدفمند براي رسيدن به آرامش در زندگي جديد» بود. نتيجه‌گيري: درك واقعيت پيوند و چالش‌هاي آن، اثرات گذرا و ماندگاري بر زندگي بيمار خواهد داشت. در اين فرايند، پرستاران به عنوان يكي از اعضاي مهم تيم مراقبت مي‌توانند با تدارك برنامه‌هاي آموزشي مناسب و مراقبت‌هاي حمايتي، بيمار را در يادگيري تطابق با كليه پيوندي و چالش‌هاي آن ياري رسانند. كليد واژه‌ها: فرايند تطابق، تطابق، پيوند كليه، نظريه داده‌بنياد

Background and Objective: Kidney transplantation is considered as selective and the most effective alternative way in treatment of patients with end-stage renal disease. Transplantation may improve patientsʹ life quality, but it can also lead patients to face new challenges and concerns. Since there is little information about coping process of patients with renal transplantation, understanding how they cope with transplanted kidney may help health care providers to plan and implement appropriate and holistic care programs. The aim of this study was to explain the coping process in renal transplant recipients. Materials and Methods: A qualitative method with grounded theory approach was used to explain renal transplantation coping process. First, participants were selected based on purposive sampling and among renal transplant recipients.this process continued gradually by theoretical sampling. 11 renal transplant recipients in Afzalipour hospital, Kerman, Iran, and 3 family members were interviewed (n = 14) during 2011-2012. Data analyzing was done followed guidelines proposed by Strauss and Corbin (1998). Results: Three categories emerged from data analysis including "perceive reality of transplantation", "contrastive strategies" and "learning challenging life". Results demonstrated transplanted kidney coping was a slow and steady process with special characteristics in each step. The main theme emerged from the text was continuous and purposive struggling to reach calmness in new life. Conclusion: Understanding the reality of transplantation and its challenges had either temporarily or permanently effects on patient’s life. In this process, nurses as important members of health care team can provide appropriate educational programs and supportive care in order to assist patients to learn how to cope with transplanted kidney and its challenges.
