بررسي الزامات استقرار منشور حقوق بيمار در مراكز درماني كشور: يك مطالعه دلفي

Surveying the prerequisites for establishing patientʹʹs rights charter in health centers of Iran- A Delphi study

زمينه و هدف: حقوق بيمار يكي از محورهاي اساسي حاكميت خدمات باليني و نيز از شاخص هاي مهم كيفيت خدمات ارايه شده در نظام سلامت است. حقوق بيمار در ايران در منشور حقوق بيمار بيان شده است كه اجرا و رعايت آن نيازمند بستر سازي و فراهم كردن زمينه هاي لازم است. هدف از اين مطالعه بررسي پيش نيازهاي ضروري و تدوين سندي جهت استقرار منشور حقوق بيمار در مراكز درماني كشور با استفاده از آرا خبرگان بوده است. روش بررسي: مطالعه با استفاده از تكنيك دلفي انجام شد و 30 نفر از صاحبنظران حوزه حقوق، پزشكي و اخلاق حرفه اي كه به صورت هدفمند انتخاب شدند در 4 دور مطالعه شركت كردند. در دور اول سوالي راجع به راهكارها و الزامات استقرار منشور حقوق بيمار در مراكز درماني كشور ايران پرسيده شد؛ سپس پاسخ هاي اين مرحله، استخراج و در يك قالب 7 گانه تدوين و وارد مراحل دلفي گرديد. نظرات شركت كنندگان با استفاده از مصاحبه حضوري و پست الكترونيكي از فروردين 1391 تا شهريور 1392 جمع آوري شد. سند نهايي در پايان دور چهارم تدوين شد. يافته ها: منشور حقوق بيمار در 7 حوزه شامل آموزش دانشگاهي، پژوهش، نظارت، مديريت فرآيند، فضاي فيزيكي، سازمان و مديريت نيروي انساني و 82 گويه دسته بندي و تنظيم شد. از مجموع 178 گويه در معرض قضاوت صاحبنظران، 51 گويه (8/34%) مورد توافق نهايي صاحبنظران قرار گرفته و در سند نهايي وارد شدند. نتيجه گيري: نتايج اين مطالعه علاوه بر اينكه حوزه هاي نيازمند تحول و فرصت هاي اصلاح به منظور بهبود رعايت حقوق بيمار را مورد شناسايي قرار داده است، نشان داد كه بسياري از اهداف منشور حقوق بيمار مي تواند با اصلاح فرآيندهاي سازماني و توانمندسازي نيروي انساني و بدون صرف هزينه فراوان محقق شود.

Background and aims: Patientʹs rights are one of pivotal elements of clinical governance and important indicators of quality care in health area. Patientʹs rights in Iran are stated in patientʹs rights charter and its establishing need to lying plots a set of prerequisites. The aim of this study was to survey prerequisites and to develop a bill for establishing patient rights in Iranian health centers using expertʹs ideas and comments. Methods: The study was conducted using the Delphi’s approach, and 30 purposefully selected participants include experts in law, medical groups and ethics, as panelists were enrolled in 4 rand Delphi technique. In first rand, the ideas of panelists in response to question" what prerequisites are needed to establishing patients rights in hospitals" were collected and then a bill including 7 domain was developed and then entered in Delphi process. The comments of panelist were collected by E- Mail and face interview through April 2012 to September 2013. Final document was developed in the end of 4th rand. Results: A consensuses bill for establishing patient rights was main result of this study that developed in 82 items in 7 domains including: academic education, research, supervision, process management, building, organization and manpower. At al, out of 178 items that judged by panelist, finally 51 items (34.8%) consist many suggestion were agreed and entered in final document. Conclusion: Results of this study recognize the areas which need to takeover and correction in order to improve patients’ rights observance. In addition, the results showed that goal of bill of patients’ rights can be achieved with minimal cost and changes such as correcting organizational processes and empowering staff.
