زمينه و هدف: تنگي مجراي ادرار در مردان يك مشكل شايع است. عمل اينترنال يورتروتومي‌با ديد مستقيم، يكي از روش‌هاي شايع درمان در اين بيماري است. هدف اين مطالعه بررسي نقش گايدواير در تسهيل عمل يورتروتومي داخلي با ديد مستقيم بود. روش بررسي: در اين مطالعه آينده نگر، 84 بيمار با تنگي مجراي قدامي ‌به صورت تصادفي به دو گروه مساوي تقسيم شدند. گروه اول به روش روتين تحت عمل يورتروتومي‌داخلي قرار گرفتند و در گروه دوم ابتدا با استفاده از يورتروسكوپ ولف 8 ـ 8/9 فرنچ، گايدواير 035/0 فرنچ از سر نرم آن وارد مثانه شده و سپس اقدام به عمل يورتروتومي‌داخلي شد. داده‌ها با آزمون آماري توصيفي تجزيه و تحليل شدند. يافته‌ها: متوسط زمان عمل در گروه اول 14 دقيقه و در گروه دوم، 10 دقيقه بود. مقايسه عوارض اوليه بعد از عمل نشان داد كه در گروه اول در4 بيمار(52/9 درصد) خونريزي و عفونت تب‌دار در3 بيمار(14/7 درصد) رخ داد، ولي در گروه دوم چنين عوارضي مشاهده نشد. تنها عارضه ديرس در هر دو گروه عود تنگي مجرا بوده است كه در گروه اول در 8 بيمار(04/19 درصد) و در گروه دوم در 3 بيمار(14/7 درصد) ديده شد. نتيجه‌گيري: استفاده از گايدواير در جريان عمل يورتروتومي‌ممكن است هم باعث تسهيل عمل شود و زمان عمل را كوتاه‌تر كند و هم عوارض كوتاه مدت و دراز مدت عمل را كاهش دهد.

Background & aim: Urethral Stricture (US) is a common urolologic problem. Direct optic internal urethratomy (DOIU) is a common option for treating US. The purpose of this study was to investigate the role of the guide wire to facilitate direct vision internal in adult male patients. Methods: In this prospective study, 84 patients with anterior urethral stricture randomly divided into two groups. The first group underwent routine DOIU surgery and the second group internal urethratomy was carried out using Wolf ureteroscope 8-9.8 French and guide wire 0.035 French which inserted into bladder. Data were collected and analyzed using software (spss). Results: The mean operation time was 14 minutes in group one and for group 2 was 10 minutes. The comparison of complications after surgery showed that in the first group urethral bleeding accrued in 4 patients (9.52%) and fever infection was seen in 3 patients (7.14%) respectively but no such events were observed in group 2. The only late complication in both groups was recurrence of the stricture that was seen in 8 patients (19.04%) of group one and 3 patients (7.14%) of group 2. Conclusions: The results of this study showed that using a guide wire during DOIU may facilitates the procedure; shortens the operation time and reduce the effects of short and long-term operation.
