Nurses and Physiciansʹʹ Viewpoint toward Interprofessional Collaboration

زمينه و هدف: همكاري پرستاران و پزشكان، اصلي ضروري براي ارايه مراقبت‌ها است. اين پژوهش با بررسي ديدگاه پرستاران و پزشكان در مورد همكاري بين حرفه اي، سعي در نمايان ساختن شرايط موجود جهت اتخاذ تدابير و ارتقا روابط حرفه اي دارد. روش بررسي: در پژوهش حاضر كه يك مطالعه توصيفي مقطعي بود، ديد 134پرستار و 66 پزشك شاغل در مراكز آموزشي درماني تبريز از طريق شيوه تصادفي طبقه اي و با استفاده از پرسشنامه جفرسون مورد بررسي قرارگرفت. روايي ابزار ياد شده با استفاده از روايي محتوي و پايايي آن توسط آزمون مجدد بررسي و با توجه به ضريب به دست آمده (86/0) مورد تاييد قرارگرفت. داده‌هاي جمع‌آوري شده با استفاده از نرم افزار SPSS نسخه 13 و از طريق آمار توصيفي و آزمون پيرسون، Tو ANOVA تجريه و تحليل شد. يافته ها: نتايج مطالعه حاكي از نگرش نامطلوب پزشكان نسبت به همكاري بين حرفه اي در حيطه هاي سلطه گري پزشكان و استقلال پرستاران بود.اختلاف معني داري بين ديدگاه پرستاران و پزشكان در چهارحيطه سلطه كاري پزشكان(001/0 > P)، استقلال پرستاران (001/0(P < ، كار تيمي(003/0(p= و درمان در برابر مراقبت (001/0(p < مشاهده شد. در خصوص ارتباط بين متغيرهاي دموگرافيك با درك پرستاران و پزشكان از همكاري بين حرفه اي، نتايج نشان داد با افزايش سنوات خدمت، ديدگاه پزشكان بهبود يافته (42/0, r=008/0= (pدر مقابل پرستاران با افزايش سابقه كار ديد نامناسب تري نسبت به همكاري بين حرفه اي پيدا كرده بودند(38/0- , r=03/0= (pهمچنين در بخشهاي ويژه نسبت به بخشهاي عمومي پرستاران و پزشكان ديد بهتري نسبت به همكاري بين حرفه اي داشتند. نتيجه گيري كلي: با توجه به نتايج مطالعه فوق مبني بر ديد محدود پزشكان نسبت به همكاري بين حرفه اي به ويژه در حيطه هاي سلطه گري پزشكان و استقلال پرستاران، اتخاذ تدابير لازم از جمله ايجاد برنامه هاي آموزشي مشترك بين دانشجويان پرستاري و پزشكي جهت بهبود همكاري بين حرفه‌اي توصيه مي شود.

Background and Aim: Collaboration between nurses and physicians is essential for providing primary care and it is key factor for facilitating empowerment and rehabilitation of patients. This study by determining viewpoints of nurses and physicians toward professional collaboration tries to determine the existing conditions and to fasilate collaboration between nurses and physicians. Material &Methods: This cross-sectional study assessed the viewpoints of 134 nurses and 66 physicians regarding interproffeshional collaboration in teaching hospitals of Tabriz-Iran. The data were collected by Jefferson questionnaire. Its reliability was 0.86 by using test- retest and its validity was received by content validity. Data was analyzed by SPSS-PC(v.13) software and using of pearson, One-way ANOVA and independent T test. Results: The results showed that the physicians’ attitudes toward profesional collaboration was not so good in physician’s dominance and nurses’ autonomy subscales. There were significant differences between the nursesʹ and physicians’ viewpoints in the subscales of: physician’s dominance(p < 0.001), nurses’ autonomy (p < 0.001), teamwork (p=0.003) and caring as opposed to curing (p < 0.001).Assessing relationship between demographic variables of nurses and physicians and their viewpoint toward interprofessional collaboration showed direct relation between physicians’ viewpoint and their experience(p=0.008,r-0.42) but, this relationship was reversed in nurses and their viewpoint had been worse with increasing experience(p=0.03,r- -0.38). Nurses and physicians’ viewpoint toward interprofessional collaboration in I.C.U and C.C.U wards were better than general wards. Conclusion: Regarding the undesirable attitude of physicians in the physicians’ dominance and nurses’ autonomy subscales, providing training programs for nursing and medical students about professional roles with emphasis on professional communication skills during their educational courses is recommended.
